Don't know if someone said it already. But No one can hear Saiki's thoughts unless he uses telepathy (which is very rare). His responses are an internal monologue that no one else can hear.
I wanna say that first but for some reason,i couldn't.

No one can hear Saikis thoughts only us. When people talk to Saiki he uses telepathy and also uses his powers to make it seem like the way he speaks is normal even tho he mouth doesn't move. It's never really explained but it's funny.
There are rare, like really rare occasions he opens his mouth to speak if you can catch them

As for saiki’s telepathy thing he makes it seem normal to others but only we can notice he’s using his power to communicate.
But in the last episode there’s a scene where after saiki saves the world and he meets all his friends he actually laughs(?) and talks to them like a normal person without using his telepathy, and everyone somewhat notices its different. It was a nice touch of detail.
