A cherrypicker is usually the one with the basket for one guy or maybe a platform. Sometimes, you will hear them using a "pettibone". It's pretty similar, but not quite. Forklift + cherrypicker = pettibone.
That is called Depth of Field. What you need is a camera with a manual focus on your lens and a low f-stop. So on a DSLR, a lens that has a very low f-stop would be around f1.2. Hope that helped.
I'd like to see how the stabbing worked in old movies, for example in Zulu (1964) where technology was much more primitive, and yet they realistically show bayonets going into a topless person's skin and blood squirting out. How does that work?

@FoodHealsYou autodesk matchmover? mocha (came with AE CS5 and above), boujou, syneye, pfttrack. pick your poison (but prepared to spend some serious money or bandwidth if you dont have CS5 or CS5.5 already for mocha)
Ryan, you can hide the blue dots of the track with a plugin called "Dust and Scratches" It works fine :) Just mask around the dot and skin, and play with the 2 options of the plugin... It worked fine in my project :D
instead of having to hasstle with tracking and animating..couldn't you have just put the stick next his his neck? At that angle, being next to or through has no visual difference. Kinda like the sword through your arm pit to look like it went through you. Just something to think about.

The background gets blurred when you have a larger aperture. This is easy to achieve with for example a DSLR like Canon EOS 60D, 600D, 7D or 5D.
