Go slow on sugary fruits and avoid snacking between meals. Avoid sugar and junk too. time, the weigh will start going down. Don't wait until you have 900k to go for this procedure. On the other hand I've seen 65 year olds who look 40 just from consistency in their diet and exercise. It requires a lot of saying no to delicious foods and going out to exercise when you are not in the mood. Young people are dying from heart attacks and lifestyle induced diseases such as type 2 diabetes. We need to find the motivation to keep healthy. Surgery is always a risky option. I too gained a lot of weight over the pandemic but I made a conscious decision to put my life in order. I begun by setting weekly running goals that I revised regularly. I could set a target of say 20kms which I'd spread over several days. I made sure that I hit my weekly targets. When I started I could barely run a distance of 3kms without running out of breath but with regular progress I can now run 10Km with little effort. Please find some exercise that you enjoy and be consistent. Include physical exercise in your daily plans. Even if it is dancing. Start and don't stop. Fasting works..drop the carbohydrates too...Weight is managed on the plate, exercise is for better blood circulation.
