What about the neurovascular bundles in the digit, in a chronic case? When we try to extend the PIP surgically, is there a risk of vascular compromise by stretching of the neurovascular bundle? What do we do at that time?

Very helpful. For early stage boutonnière deformity in rheumatoid arthritis, does splinting actually help straighten out the finger or does it help prevent further deformity or is it only to help with discomfort?
At 15:20 you said if surgery is permitted only if you have full passive range of motion at PIP joint. However what if the adhesions is stopping passive range of motion. I had a previous surgery where they k wired pip joint with open reduction, now the joint doesn’t flex much and also dip joint is stiff. However when i try to push finger down passively , I can see tendon bulging at the back of my hand which doesnt happen on my good fingers. Am I right to assume this means tendon is stuck and surgery will fix it.
Yes, in some situations when therapy is not able to get supple joints surgery can be done, but the results may not be predictable

If you have had surgery for this deformity but your knuckle bone collapsed causing the finger to drift can you have the surgery again?

Dr, I had a hand surgery… my ulner nerve and artery were sutured… this is 5weeks now and I can’t make a fist or extend my fingers… also my palms at rest makes a claw sign… please what do I do???
