  • Dialysis care providers tend to focus just on the numbers, but trying to make the numbers perfect may be causing more harm than benefit. In this video, I give real talk about when and how to stop (or at least change) dialysis.
    I say more on my website blog: www.thenephrologist.com/blog/2023/2/12/so-you-think-you-want-to-stop-dialysis
    Check it out!

    Time stamp 8:47. Stopping dialysis ahead of time. A lot of doctors seem to focus on numbers instead of the patient. If more doctors could understand not all patients are the same I believe there could be a better understanding and the bridge of communication could be more open.

      Everyone telling them to not give up, well you try living their life and have their mentality then tell me if you won’t want to do the same… what I’m trying to say is never delude people into believing life gets better if you know for a fact it won’t

    My boyfriend has been on dialysis for 26 years and still has the same fistula. His body has been through a lot of different changes. Are there any holistic ways to stop his advanced calcification?

    What about if your 40s and you do hemodialysis at home and makes you so sick afterwards? Kidney function 15-18% still urinating like a regular person. Is wise to get off and monitor it without doing dyialsis until you get your transplant?

    Thank you for posting again Dr. Vanessa. I fully understand the pitfalls of giving medical advice without having access to a patient's full history; but I need to know (for research purposes only) what was the solution for the man with incessant itching? I suffer from this and it is maddening, phosphorus is controlled, I use benadryl and slather myself in hydrocortisone, my dialyzer is flushed, I only use gentle cleansers on my skin and clothes but nothing works.

      Gabapentin in very small doses in patients on dialysis. It’s often given to people with diabetic neuropathy up to 900mg 3x a day, but I once had a dialysis patient who slept some 18 hours with 100mg pill (smallest pill size) 😮 so I had to order the liquid form!

    • @mikerossgup
      @mikerossgup Год назад +1

      Thank you for your response Dr. Vanessa you are a gem! Your answer is a bit funny to me personally as I was prescribed gabapentin for neuropathy in my feet and ankles. I read the insert (you know the one written in microscopic text folded and stuck on the top of the bottle) and decided against taking it due to the side effects. Well my search continues. Thank you again.

      @@mikerossgup You’re welcome, but keep in mind every medication has a long list of side effects, that the vast majority of folks never get. For gabapentin, the most common are sleepiness and nausea, which is why we have people start it at night. I encourage patients to be open to seeing if the benefits outweigh the downside.

    • @realkidneytalk
      @realkidneytalk  Год назад

      Sickle cell trait alone usually doesn't cause problems. Could something else be causing your severe anemia? Everybody on dialysis has low hemoglobin. It's end-stage kidney disease that kills people; rarely does anyone die directly because of dialysis.

    • @realkidneytalk
      @realkidneytalk  Год назад

      Planned to respond in the video I just posted but it was getting too long! Depends on the cause...just dry skin, phosphorus. For the patient I mentioned in the video gabapentin worked beautifully. But WARNING: Sometimes 100mg can be dangerous for folks with end-stage kidney disease. Please discuss with your doctor.

Stress can cause blood pressure to rise and that's Not! good for your kidneys not to mention other physical issues!

    Every patient has autonomy when making medical care decisions. The conversation should start with WHY start dialysis in the first place. I have made the life choice NOT to start dialysis in the first place. Complications, poorly trained staff and medical errors with no apparent accountability are valid reasons to be wary of starting in the first place.
