I have few important queries sir. 1.NS at a rate of 30ml/kg is too high even when the patient is in shock as it can exacerbate pulmonary edema due to pre-existing myocardial dysfunction.A 5 to 10ml/kg in 20 min should be appropriate. 2.Regarding the inotropes,rather than dopamine and noradrenaline,Dobutamine is the vasopressor of choice as it is physiologically suitable (in improving myocardial contractility)..Notable exception is hypotensive shock with bradycardia where adrenaline is the choice... 3.I want to know your experience with ASV as it is effective only in initial 30minutes to 60minutes.What is the cost of that?and given the high success rate of prazosin and narrow treatment window and high cost of ASV .. Do you still recommend ASV to patients?
1. Recommendations for shock management is 30 ml/kg fluid bolus Use you probe and look for ivc and myocardial contractility If there is evidence of heart failure start with 10 ml/kg 2. Dobutamine is an inotrope, its not the preferred agent if BP is less than 90/60 as it can worsen Hypotension 3.Anti Snake Venom(ASV) has no role, but Scorpion antivenom availability is low and it has no additional benefit as comapred to prazosin
For all patient,should we give prazosin?or nly symptomatic..I saw the patient who had hypertension 140/90 and tachycardia..no other complaints..should we start prazosin in this condition?
