  • MRI For Fibroids: Case Study
    Some patients wonder why they need to undergo a pelvic MRI when they have had a pelvic ultrasound showing fibroids. While ultrasound can diagnose fibroids, it significantly underestimates the number of fibroids due to a much lower resolution than MRI. Occasionally, patients with adenomyosis will be misdiagnosed on ultrasound as having fibroids.
    In this video, Dr. John C. Lipman of Atlanta Fibroid Center demonstrates the difference between ultrasound and MRI imaging and explains why MRI is the gold standard for accurate fibroid and adenomyosis diagnosis.

    Some women have an aversion to a transvaginal ultrascan and very little is done to promote the pelvic MRI scan. In the UK, doctors have to follow guidelines and go down the route of smear, TVUS, biopsy etc. I managed to find a private specialist who listened to my concerns and was able to get a pelvic MRI. Will schedule to do this in the coming weeks. I still may need to get the TVUS, biopsy depending on the results. But if the MRI is negative I see no reason to have other uncomfortable invasive tests. Right now I am just relieved to know Pelvic MRI exists.

      When my GP suspected fibroids (because of my severe anaemia and mention of heavy periods (that I thought were normal!), I was scheduled for ultrasound and was offered to decline transvaginal and only have transabdominal if I preferred. I was happy with both (it wasn't painful nor too uncomfortable) but I still had to go for an MRI scan to determine what kind of surgery will be best to remove the fibroids.
      Perhaps ask to have only the transabdominal ultrasound? I believe it will be enough to see if you have fibroids and whether they are ok to be left alone or will need a further look at with an MRI scan.

      @@katarinajanoskova Thanks. They are trying to find the cause of my anaemia. Periods are normal but had one episode of heavy bleeding. I had an abdominal pelvic ultrasound when I declined the TVUS. It was not clear enough and that't when I felt pressured to have the TVUS. Anyway, the MRI is scheduled for next week. I will never have a TVUS that's how opposed I am to that.

      @@kaxar6954 Good luck with the MRI. It's a very concise and clear imaging technique and completely non-invasive, so you'll be fine. Just close your eyes! (Or at least it helped me loads.)
      I wish I went to a GP much sooner instead of thinking the worsening periods are normal with getting closer to 40.

      Is pelvic MRI with contrast even necessary before the operative hysteroscopy? They saw some polyps during the diagnostic hysteroscopy and im asked to do MRI with contrast but im afraid of the side effects of gandolinium to my body and for my fertility journey (ivf)

      @@jodettehattori4177 I had my MRI last month and I told them I don't want contrast. I also refuse the offer of meds to relax my muscles. I was not forced to have my MRI with contrast. It went well, a bit long. Make sure you keep very still and follow the instructions when they tell you to breath in and out.
      I would not want to do another one in a hurry.

    Yes! MRI Ultra Sound wasn't definitive.Recommended MRI for clarification 1:29 .

    I had a leep procedure last April I got told it was a fibroids. Then early this yr I felt lump again n I decided to go n get it checked. Since I moved to different state I had to see different OBGY. The entire time wherever I've visited I only met with the nurse not the doctor herself. Then recently I noticed I tend to bleed a little during the sex I go bk for a visit n I get an order for ultrasound n MRI. Again no doctor meets with me but the nurse n I get told there's an unusual mass they see in MRI n sent me to Hematology-oncology for biopsy. When I made a visit the doctor himself does test as usual looks n feels inside tells me it doesn't feel like a cancer but we still need to go for biopsy and oh he even brought up hysterectomy.

    Which would you recommend prior to a consultation for UFE, an MRI with or without contrast or both??

      For initial consult pre-UFE a pelvic MRI (T2 imaging in sagittal, axial, and coronal planes) without contrast is all you need. For 3 month follow up MRI post UFE, a pelvic MRI (T1 & T2 imaging) with and without iv contrast is necessary.

    Which MRI would you recommend to evaluate if I'm a good candidate for HIFU, UAE or uterine fibroid ablation? T1,T2, w/wo contrast? Thank you!! I'm getting MRI this week.

      Ablation does nothing to the fibroids and shouldnt be performed for fibroid-related bleeding. HIFU doesn’t treat all fibroids and very niche and not covered by insurance. UFE should be able to help. For the MRI, T2 imaging in Sag, Ax, and Cor planes is all that is needed. Good luck. Learn more at ATLii.com

    Hi Doc, Can ultrasound measure the size of the fibroid?

      It can but not as reliable as pelvic MRI. Ultrasound underestimates the number of fibroids present and it often fails to diagnose adenomyosis when its easily seen on MRI.

    My Gynecologist wants me to get a pelvic mri, but I just can't do it. I attempted, but had to abort mission 5 minutes in. So, now she said as an alternative I can do a hystonogram ultrasound.

      The sonohysterogram only tells you about the uterine cavity and that does not replace a pelvic MRI. Perhaps you could an open mri. They were designed for claustrophobic people. You could even have sedation in some centers to help you. Good luck

      MRI is not typically needed before hysteroscopy. I would ask your Gynecologist the reason he/she is needing this information.

      @@Atlii Thanks for the reply. Im not really sure why the doctor suddenly wants an MRI with contrast. I was asked by my fertility doctor to visit a bigger hospital with an OBGYNE department to have it checked after he saw some polyps after the diagnostic hysteroscopy (my ivf doctor said he couldnt do operative hysteroscopy) I dont have any complaints, no heavy bleeding symptoms, no endometriosis symptoms, no fibroids shown after the diagnostic hysteroscopy, etc. When my fertility clinic referred me to the bigger hospital I was expecting that i would be scheduled for the operative hysteroscopy due to the polyps seen after the diagnostic hysteroscopy. Im not really sure what the other doctor from that bigger hospital wants to see. I just want the schedule for the surgery so that I can have my embryo transfer after surgery. I have to cancel my FET because of this MRI which for me doesnt make sense.
