I have a very similar issue. Replaced one cd3215 which went from 1.6 to 2.6 V. Destroyed again? Did you ever find the issue on that board? I have 3 ports taking 5v and around 160 m AMps and one that is 5v and like 0.009 A - that one actually has 3.3V so it seems to work ok.

I found if no 1V1 change the 3215 !
