- In this video Dr. Angelish Kumar, a New York-based urology specialist, discusses the following:
● Menopause is signalled by 12-months since last menstruation. Perimenopause is the 10-years prior to this and can include fluctuations in estrogen levels. These fluctuations can cause systemic symptoms such as hot flashes, vaginal dryness, sleep disturbances and/or irritability.
● These estrogen fluctuations can also be the reason that some women will develop UTIs for the first time in their lives during perimenopause.
● The genitourinary syndrome of menopause (GSM) symptoms progress over time. These are the multiple changes occurring in the external genitalia, pelvic floor tissues, bladder and urethra, and loss of sexual function and libido.
● The vagina needs estrogen. Vaginal estrogen is different to hormone replacement therapy. It is a topical application that is suitable for almost everyone and should be commenced as soon as any of these GSM symptoms arrive.
● Because vaginal estrogen changes the architecture of the cells in the vagina, therefore promoting lactobacillus, it can take up to 3-months to see results and should be used as an ongoing maintenance tool to combat any lower urinary tract conditions.
● Depending on a patient’s individual situation, Dr. Kumar may use testosterone, progesterone or prasterone in treatment.
● It is possible for changes in the urinary microbiome while menstruating to influence when patients get a UTI.
● When determining the cause of a patient’s symptoms, enquiring about the past effectiveness of antibiotics can sometimes reveal clues that the cause is not bacterial.

About Dr. Kumar
Dr. Kumar is a board certified Urologist who graduated with honors from The Tufts University School of Medicine and is a member of the prestigious Alpha Omega Alpha Honor Society. She completed her urology training at NYU Langone Medical Center in New York City. In 2017, she was the recipient of the gold star teaching award from the Albert Einstein College of Medicine. In 2020, she founded Women’s Urology New York, a unique practice focused on providing compassionate and comprehensive urologic care exclusively to women. In addition to being a diplomate of the American Board of Urology, Dr. Kumar is a member of the American Urologic Association, Society for Urodynamics and Female Urogenital Reconstruction, and International Urogynecologic Societies. She also serves on the Women’s Health Executive Committee for Springboard Enterprises.
