My theory is this was Withered Foxy back to when the previous location was still open. The animatronic itself witnessed the kids deaths and it messed him up, which is why the Phone Guy says "he's always a bit twitchy".
The Missing Kids died in the Foxy Go! Go! Go! minigame in FNaF 2 When you play three times as Foxy in Go! Go! Go!, your objective is to simply make your way from Pirate’s Cove to the room next door where five small children stand to cheer upon your arrival with confetti. But when you play the minigame a third time however, as you move from Pirate’s Cove, you can see the fuchsia figure standing at the bottom left of the screen. That person is indeed William Afton, also known as The Purple Guy. And when you get to the next room for the final time, you find the same five children sitting slumped down, gray and dead, followed by a finishing jumps are from Withered Foxy. I believe that after William lured the children into the back room, he went next door to Pirate’s Cove to tamper with Foxy to have him murder the poor kids. The newspaper article in FNaF 1 stated that the worker was caught on camera wearing a cartoon mascot costume to gain the children’s trust. During that time, William made Foxy go to the back room to kill the children the second time you played him in Foxy Go! Go! Go! Then during the third play, William is seen next to Pirate’s Cove, smiling and without a costume, before Foxy finds the dead children that he mirdered. I also believe that William was worried he might leave fingerprints on weapons he would use to kill the kids and that he would be charged. So instead, he messed with Foxy’s wiring and used him as a silent but deadly killing machine. My speculation is that the Foxy Go! Go! Go! minigame is the murder of the children that happened on June 26th, 1987. The first time playing Foxy Go! Go! Go!, you normal walk next door to surprise the children. The second time playing the minigame, the same thing happens, but I think that was when the killing began. So when you played for the third time, the kids were already dead and the deed was done. That’s where the jumps are comes in. But of course, that’s just my FNaF theory.

I don't think there is (if you're referring to the bottom-most kid). I think it's just a sode-effect of the scanline effect that makes the smile harder to see

Mini-game mean if Foxy saw five deaths, there would have been 6 murders, and of these happened at Fazbears, then Charlie couldn’t be one of the ghosts here, so could Foxy actually just have gone haywire in the REAL timeline with Five Nights At Freddy’s 1-SB? Or could it just be a retcon? But Scott had only actually retconned the series once, and why would it be so simple as a mini-game? Wouldn’t it be the retcon for Dream Theory that we got in SL? How about this, the fifth kid is the one who possessed BB, that could be a real explanation for Foxy going haywire, it would even explain why he’s so much more aggressive as he’s not really possessed, he’s just crazy. I went on for a while, but I honestly think Foxy is just insane, and he’s also out of order in 1, which would also fit. If you read this whole thing, THANK YOU. I actually spent a bit of thinking on that and I feel like a bootleg MatPat considering how stupid this theory is, but I still think it might be canon. This leads me to believe that this minigames only meaning is that Purple Guy is the one who committed the murders. You don't know that as a player until you see this scene. You could argue that this is unwithered foxy. Then I present another counter argument: The murders happened in the safe room. Phone Guy mentions in FNaF3 that the Springbonnie animatronic has been noticeably moved and advised that the safe room should not be used to hide. And "never should a customer ever be taken into this room and out of the main show area". Guess who did exactly that?
