The inserts that came loose are called pop nuts, They are mounted with a special tool that expands part of it in the frame tube. I have used a lot of these and if mounted correctly they should sit there forever.🙂

Obviously they were not , one of then the bolt was actually stripped in the thread. But it’s all good now .
