thank you. I'm gonna be honest the challenge is less about being good at the game and more finding ways to ignore as much of it as possible and being lucky. many runs lost due to poor level geometry that didn't go hand in hand with the knightmare settings, or just plain bugs
it is an insane thing to have but you have to hope that you make it to the grub pit, gastronomole shows up in the first level, actually has the item (he doesn't require having the white smoke though) and you have the gold for it in the mode where prices are more than twice as high as normally
The gist is that in Knightmare he often has the black smoke and makes smeltwork flamethrower noises, which means worse, lackluster healing items, but still has a rare chance of showing up like normal. Grub Pit I is the one instance I found where he can have the black smoke and still sell good items
First time seeing that Black knight fight... Boy, there's sure some pretty cheap stuff with the moving platform. Thank God I didn't get the DLC, this would have made my blood boil
Minimize randomness and risk in any way you can. Don't bother with siderooms or pointless gold collecting (unless you're going for true ending), and otherwise just keep doing it until you are solid at it. I used Brash Bracers because the damage is good, the first hit you take protects you from getting instakilled by anything, even the saw, and you technically get healed each level with it, so if you are confident you can survive, you can focus on using the cogs to get upgrades more than anything.
You have my utmost respect for beating Knightmare 10! Still kinda wish we got more lore on Black Knight, but the boss fight looks super fun.

replaces your health with tons of shields
