Hi, These are different forms of the word "lekarz" (a doctor).
"Lekarzem" is a form in the instrumental case (narzędnik).
Jestem lekarzem. - I am a doctor.
"Lekarza" is a form in the accusative (biernik) and dative case (dopełniacz).
Znam dobrego lekarza. - I know a good doctor.
Szukam lekarza. - I am looking for a doctor.
