This is a cool headcanon and I'm sure Dream is great with ocarina and this tool was clearly accessible and easy to get.
As for hallucinations, I always had a headcanon that before the incident and when Nightmare allowed himself to be bullied, he could see and hear hallucinations associated with the spirit of the murdered person by Nim and because of this, his problems with insomnia also began (bullying, hallucinations, in general hatred). Still, this spirit is to blame for og Nightmare ate the apple (I also always had a headcanon that dreamswap Nightmare said 'fuck you all' and just went against the system like a true rebel lol, even if it made him closer to a psychopath unfortunately)
