I first heard it at Moshav Modiin for Havdalah in 2001 when we were visiting, sung by someone whom I think was originally in the Diaspora Yeshiva Band, as far as I know they sung it every week, very nice. We were all outside and it was an amazing feeling.

With Hashem's help may we all be together again soon.

It was a campaign to hold havallah celebrations all over the world for the same shabbat. So while it was the same evening, these events were held at differing times. Hence why in the singing they don't use God's name but instead say HaShem.
In 2015, motzei Shabbat on October 24th is the beginning of the yahrtzeit of Yitzhak Rabin. Shabbat itself is, of course, the yahrtzeit of Rachel Imenu.
