How to fill out Form W-9

- If you've ever worked for someone as an independent contractor, opened a bank or brokerage account, sold real estate, or applied for a mortgage, you've seen Form W-9, which is used by businesses to collected Tax Identification Numbers to either report income (or potential deductions or credits) to the IRS. Despite a recent revision of this form by the IRS, there are still areas of confusion when it comes to filling out this form.
This video shows you what Form W-9 is, how it's used, and gives several examples of the correct way to fill out the form for individuals, businesses, and other entities.

Hello! I have a single member LLC and I elected to be treated as an S-Corp for tax purposes. Does this exclude the LLC from being a disregarded entity?
Yes, once you elect a corporate tax treatment for your LLC, the IRS no longer considers it to be a disregarded entity.
LLC 1 member can i put both ssn& ein?
No, you provide one or the other.
What if I have a Schedule F instead of Schedule C? Does 4:05 still apply to me?
When it comes to filling out W-9 Forms, Schedule F treated the same way as Schedule C

What if I just need to change my name after a divorce and the form is needed for verification?
Just fill out a new form and hand it in. Just make sure you have changed your name with the Social Security Administration.
