How to fill out Form W-9

  • If you've ever worked for someone as an independent contractor, opened a bank or brokerage account, sold real estate, or applied for a mortgage, you've seen Form W-9, which is used by businesses to collected Tax Identification Numbers to either report income (or potential deductions or credits) to the IRS. Despite a recent revision of this form by the IRS, there are still areas of confusion when it comes to filling out this form.
    This video shows you what Form W-9 is, how it's used, and gives several examples of the correct way to fill out the form for individuals, businesses, and other entities.

    When sending a W-9, do all pages of the form need to be summitted, or just the first page (information and signature)? Thanks in advance

      Just the first page is needed, the other pages are instructions.

    Hello! I have a single member LLC and I elected to be treated as an S-Corp for tax purposes. Does this exclude the LLC from being a disregarded entity?

      Yes, once you elect a corporate tax treatment for your LLC, the IRS no longer considers it to be a disregarded entity.

      No, you provide one or the other.

    What if I have a Schedule F instead of Schedule C? Does 4:05 still apply to me?

      When it comes to filling out W-9 Forms, Schedule F treated the same way as Schedule C

    What if I just need to change my name after a divorce and the form is needed for verification?

      Just fill out a new form and hand it in. Just make sure you have changed your name with the Social Security Administration.
