  • Deciding to have ACL surgery can be intimidating and overwhelming with all the decisions that need to be made, such as which surgeon am I going to go with, what is the best graft to use, who/where am i going to rehab from this surgery, and how long will I be out for. In today’s episode we are here to help you become more informed and educated by going to go over the common grafts, the pros and cons of each, and general timeframes.       Thank you again! I heard a lot of bad things about allograft so l was a little bit skeptical about it, but you gave me a lot of good info, also l’m starting to understand that a lot of it has to do with the rehab, Apparently if you have a good rehab you should be fine and like you said trust the process.     I had a ACL recon using Hamstring tendon. It's been 4 more months now. The rehab is going pretty good. Surgerical leg Quads are about 75% strong in comparison to non surgical one. Whereas the Hamstring is a little weaker than that. Working a lot on my rehab I think so I will be good in 9-12 months. Irrespective of the graft . Your rehab can get you back to sport irrespective of which kind of autograft you used. I have heard people making new PRs in track & field after the surgery after 2-3 years or so. Common 💪🏻

      I had ACL reconstruction with hamstring tendon and ended up retearing 17 months later returning sport. going for a Quad graft this time around. hope your recovery is going well

    I’m 28 and I recently just had surgery for my ACL and meniscus they used an allograft for my ACL I was a little nervous/skeptical because I am still very active and I still play basketball often as well as train other basketball players but this video has definitely put me more at ease thank you.

    Talk about ACL Primary Repair using an Internal brace. I had both of my knees operated on 6 years apart. 2018 with ACLR using BTB patella graft and partial meniscectomy (took me about 2 years really to fully recover and stopped wearing my knee brace). My right knee was 3 weeks ago using the primary acl repair and my meniscus was stitched up as well. 2 days later I was already fully weight bearing and off the crutches with a slight limp still of course. At 3 weeks out, Im already doing body weight squat and my ROM is almost back to normal already. What a world of difference. We will see how strong this new procedure will be tho down the road. Although, theres been a lot of athletes who underwent the same procedure as me and was already back to playing sports 4-6 months later.

      Great point, and thanks for sharing your journey. We do see ACL repairs, but it is way less common and surgeon dependent. Rehab outcomes we've seen have been solid and shorter than the traditional ACL-Reconstructions, but circumstances have to be pretty ideal for surgeons to choose to do a repair

    I’m 22 and had surgery a month ago today I play basketball and I would recommend the quad as the graft the only con with it is that is has the slowest recovery time however it is shown to be the strongest even stronger than the patellar tendon. And you can kneel and lunge easier for sports stretching.

    Thanks for video getting acl surgery next week after tearing it during basketball. I think I’m going with quad. It is newer so my doctor said not much research behind as the other but seems good . Thanks for the video still not a ton of quad tendon videos.

      Sorry to hear about the ACL injury but yea we have seen a lot of quad tendon grafts recently after ACL and they have been crushing it! the most important part with that tho is the rehab! Make sure you do your rehab, let us know if you need any help

      @@TheBasketballDoctors what sucks is i stop doing leg a few months before to allow my patella tendinitis to heal so my leg have extra atrophy now. Do you have any recommendation for a PT or PT center in Phoenix Arizona ?

      @@alejandrocastillo8883 oh man yea ideally we want to be as strong as we can before surgery to improve our recovery but you will do fine. The one place I would recommend is EXOS in Phoenix they are good and if you cant find any contact us we do online rehab as well

    Im 30 and was told by my physician that he recommends allograft( kadaver). I played rec soccer 1-2 days a week. Based on what you said I should be good to play since its only mid level and not high level.

      great question! Honestly an allograft is great as long as you do the full complete rehab! The rehab is more important than the graft in our opinion

    There is higher rate of osteoarthritis in the knee when you use a bone-patellar-bone graft. Something to do with the way the quadriceps load changes after that portion of tendon is removed. The recent data shows that hamstring tendon graft is on par with patellar graft performance. I'm not aware of the 2 to 4 times higher tear rate. It's best to go with the graft choice that your surgeon has the most experience with because you're more likely to get a poor result from surgical error than the actual graft choice itself.
    Here is a meta analysis of nearly 50,000 surgeries and a quoted result from the paper "Results: Two hundred twelve of 7560 (2.80%) bone-tendon-bone grafts ruptured compared with 1123 of 39,510 (2.84%) in the hamstring group (odds ratio = 0.83, 95% confidence interval, 0.72-0.96; p = 0.01)."
    So it's only 0.04% worse than a patellar graft.

    You can find this study on PubMed and can access it for free with a simple google search.
    Regardless of graft selection, the most important thing you need to do as a patient is work very hard to rehabilitate properly. It is not a race or a sprint, but rather a long and dedicated marathon.

    I retore my acl this year and had my first test in 2019. Since it’s a retear the surgeon proposed the Achilles allograft, and may do a lateral tendonesis with it as well. I am a freshmen in college playing soccer, and I still want to play next year but the recovery may just take longer. Would you use a quadriceps graft if it was a retear?

      Sorry to hear that.. Which graft did they use for your first one?
      At the level you're hoping to get back to, we'd suggest you ask your surgeon on his opinion of using a stronger graft patellar or quadriceps tendon. The surgeon may have recommended the Achilles' tendon allograft based on what they used on your previous surgery

    Thank you for this video, I am only 17 so which grafts do you recommend for weightlifting? I do lots of heavy squats and deadlifts so does the choice of graft make a difference in that? Also how long should I wait for full weight lifting?

      At your age and what you're trying to get back into. Patellar tendon or quad tendon would probably be the smarter options for you. You will for sure get back to full weight lifting, just need to listen to your physical therapist that is helping you with your rehab. Hard to give you an exact time frame because every person is different

    Thank you for the video Gabe, I'm at a 21 year old basketball player from India and i recently tore my ACL during a college game. I'm a pretty athletic guy with a 40inch+ vertical and i'm really concerned on what graft would be best for me. Would be really helpful if you guys could give your recommendation on which graft would be best for me. Also i was wondering if i go through rehab very well and be dedicated will i be able to get to where i was in terms of athleticsm and hops?

      Hey thanks for reaching out! I would recommend either patellar or quad tendon graft due to your age and the level of sport you want to get back to. In regards to jumping, we have seen athletes improve thier vertical jump after physical therapy as long as they are doing the correct rehab.

    Hello! I found your video from Italy because I'm searching some information about the ACL grafts types. I am 26 years old, I tore my ACL and damaged my medial meniscus. I will have surgery in December and my orthopedist suggested using the allograft technique. I'm a bit perplexed because I don't do a heavy job (employee) and I do crossfit 5 times a week with competitions 2/3 times a year. What would you suggest?

      Hi, thanks for sharing your story and great question! Allograft is a viable option for you because recovery/rehab will be slightly easier in comparison to quad/patellar tendon graft and it doesn't sound like you're doing a change of direction sport (although Crossfit is inherently a very explosive sport and high lower body demand activity). Risk for allograft is a higher risk of infection and re-tear rate in comparison to quad/patellar tendon graft, so take that into consideration as well.

    I have a question too. I’m only 16 and I play tons of basketball up till I tore my acl. My surgeon said we are going to use my hamstring tendon. I have a few questions.
    Am I going to be able to go back to playing basketball as I did before?
    I’m a quick guard that uses quick moves and quick stops and cuts. Would I still be able to do this when I return?
    Am I limited to how long I work out and train after clearance?
    As a 16 year old who plays tons of basketball, would the hamstring graft be a good choice?

      Hey Thanks for reaching out! Yea all of our athletes do return back to previous as long as they do their rehab. You really have to put in the work during rehab which will dictate how well you get back. In regards to returning to full basketball we recommend waiting until at least 9-12 months. In regards to the graft we recommend a quad or patellar tendon since that is the graft we have seen and like the most

      And for the graft of hamstring, is it just what you guys have seen or is the hamstring a graft that will also work for basketball point guards?

      @@jeffreyswutchinderol4912 Once you are fully recovered you should have no issues. In regards to the hamstring we havent seen many to be honest but the most important thing for any ACL recovery is the rehab. Finding the right physical therapist is key. Let us know if you need help with rehab

    Hey there! I'm a football(soccer) player. I had torn my right knee ACL during landing. What do you think on using Peroneous longus graft? Will it have any side effects on my ankle stability considering my sport? If peroneus longus graft is good to be taken, from which leg could it be taken (considering that I am right handed)?

      Hey, thank for sharing! Not the strongest graft unfortunately, but it will do its job for the most part. If you are playing competitively and are still young, you'd probably benefit more from a stronger graft by research, such as patellar tendon or quad tendon graft.     i had a hamstring graft 3 years back and after countless hours daily of exercise and rehabilitation i wish i had been advised of different option’s and if it was today i would have gone for a quad tendon

      Hey, great question! We have seen more surgeons doing ACL Reconstruction utilizing quad tendon graft, which allows us to see it more in rehab. Very similar results and pros/cons with patellar tendon graft. Definitely have seen less anterior knee pain with kneeling with the quad tendon graft, but similar issues with quad activation and potential development of tendinitis at the quad/patellar tendon with both grafts.
      We feel like both are very great options, but we would defer to what the surgeon is more comfortable doing. I think as time goes by, we'll see more and more quad tendon grafts, as well as, more research comparing the different grafts over the long term with pros/cons.

Any problems squatting deep on knees with quad graft or is that more common with patella graft? Do you think that those who are just concerned with maybe strength would be better not getting acl surgery and its just better for sports where they require twists and turns like basketball?

    Hey! so im actually 2 weeks away from an ACL surgery. I tore it playing basketball outside 🏀 landing on a drive. And so Its been a year to wait for the surgery but ive been prehabing my body to strengthen my legs. Im currently 27 and im very low to the ground and aggressive when i play so i know that my knee will require a lot of physical demand on defense, rebounding and driving to the basketball. If you happen to see this what would be your own recommendation?

Hard to say, we would defer to the surgeon because this is your second tear and there are more factors to consider when opting for the surgery and choosing a graft. At your age, depending on your activity levels/goals, you may want to consider trying to avoid surgery if possible. Hope that helps!

    Enjoyed the video! I sustained a severe injury playing football(soccer) Posterolateral corner inc biceps femoris tendon avulsion + ACL tear. I’m currently recovering from a posterolateral corner repair + reconstruction, with ACL reconstruction coming later in the year. They used IT band and hamstring tendon grafts for my bjceps femoris tendon re-attachment and LCL/arcuate ligament repair(not 100% sure). I was initially told the surgeons plan to use my left hamstring tendon for my right knee ACL reconstruction. I’m 24 and was previously very active, decent level recreational soccer(football) and semi professional cricket. I’m aware the injury may be career threatening but I was wondering whether the other surgery would impact/prevent the surgeons from using a patellar or quad tendon graft? Essentially if there’s a physiological reason why they’d use left hamstring tendon rather than right patellar or quad tendon?

      Hey, thanks for sharing your story and sorry to hear that you're going through such a traumatic injury! With the amount of trauma (injury and surgery) that happened on your R leg, as well as, using grafts of your R leg to repair the posterolateral corner (I'm assuming they used those structures from the R side), it would make sense on why the surgeon would choose to use the L side for the grafts. You could ask your surgeon to use your L quad/patellar tendon instead of the hamstring, considering the activity level you are trying to get back to. Hope that helps!

      @@MrNk57 Not a surgeon here. But I can tell you as in the video, since you are an athletic individual and so long as your career or profession does not involve a significant amount of kneeling and squatting you would want to opt for the patellar tendon graft. There's a reason it's considered the gold standard.

    I got a MRI the doc said i need surgery because i have a torn ACL and a small tear on my meniscus. I'm 41 and I do work out , shoot the ball around and play tag and keep away from my pit bull which should I go with

      Hey, thanks for sharing! Honestly, if you don't have any pain, don't feel restricted or unstable, you may want to try going to physical therapy and going the conservative route and see if you could potentially avoid surgery all together. If it feels unstable, hasn't progressed, or your knee is affecting your quality of life after a round or 2 of physical therapy, then consider surgery. At your age and your activity level, a cadaver or hamstring graft may be a better option for you to make your recovery/rehab potentially less painful

    Hi, I have torn my acl in left knee. I go to gym 2-3 days a week and not an active runner, just moderate. Doctor has advised btb patellar graft. I am afraid would I be suffering kneel pain or anterior knee pain for my whole life due to this graft ?

      Hi, thanks for sharing your story. If your main form is exercise is running, you would probably benefit from using a different graft to limit the amount of trauma/strain on your anterior chain. Also, you may not even need surgery if you aren't planning to do a change of direction sport and are able to rehab enough to get your knee stronger/stable. Hope that helps!

    Is the quad graft good for runners? I’m 29 and still running at a high level, I hurt it playing soccer and won’t be playing that as much once I’m back

      Quad graft is a strong graft for an active population, but we always tell our clients to figure out your goals following surgery and discuss that with your surgeon because your surgeon likely have their own preferences and are more comfortable using a certain graft. If you're not planning to get back to playing to soccer, other graft options (hamstring, cadaver) could be an option for you. Your rehab is going to be the most important part of your recovery regardless of the graft choice, and smart programming and load management as you return to jogging/running are going to be key for you!     hello guys - will be having acL surgery in a couple of days. Dr advised we'll be doing an autograft, from peroneus longus tendon (calf to ankle?)
    what do you think of this source for the garft?

      Hi great question.
      It is a muscle/tendon in the lower leg (outside calf area). We tend to defer to what a surgeon recommends and what they are comfortable with using for you because there are a lot of factors and goals that go into a graft selection (as you probably learned from this video). It is a successful graft that will make parts of your rehab easier in comparison to other grafts, but still not as strong as the patellar tendon or quadriceps tendon graft. Hope that helps!

    Hello, I tore my ACL about 6 months ago and am getting surgery next week. My surgeon recommended to me that I get either the quad or patellar tendon surgery. He said that he does the patellar the most but I don’t know which one to choose because we don’t have too much long term data with the quad. I am worried that when I become much older I am going to have pain because the patellar tendon does not grow back. I got hurt playing basketball and would like to return to playing basketball for fun. My main goal is to get as close as possible to 100 percent so which graft do you think I should choose patellar or quad. I don’t want one that will fix my acl but give me a whole bunch of other problems in the future. I am an 18 year old 6 foot 2 and 205 pounds. Thank you.

      patella route has been around much longer, but they're both equally similar. From what i heard, you get a bigger scar with patella graft.

      Hi Great question! The hamstring does regrow however we recommend you really work on the hamstring strength since it really helps out with the ACL

    I'm 20 and I have to go through the surgery in the coming week. I have a complete acl tear and also my meniscus is going to be repaired. I play football and badminton. I do not play at high level sports. I just play for the fun 5-6 times a month. Which graft would you recommend? Thank you

      Hello brother ,I have question please answer. I had ACL compelte tear and medial meniscus grade 3 injury, my surgeon said he is not available for 4-5 week so,he schedule surgery after 30 days of my injury on 17 June.     I'm hypermobile and I've read that you should avoid using hamstring graft because of this. The result? I re tore it twice + noticeable weakness of my left hamstring vs right even after 6 years. Even though both hamstrings are now affected.

      Are you still deciding on the graft?
      What is your competition level (Professional, competitive, recreational, coaching/teaching)?
      Patellar tendon is the strongest, but could give you some discomfort as you return to sport because of the potential front of the knee discomfort when you're on the ground grapping, kneeling or in deep knee flexion.

      @@jayjay80850 honestly at this age level and your training it all comes down to the rehab. The rehab is the most important aspect of the recovery. Unless your back to soccer allograft is great just know it may take a little longer rehab because it takes longer for the ligamenization

      @@TheBasketballDoctors I was concerned because wrestling and Jiu Jitsu puts joints in awkward positions. That being said I was curious why an allograft was offered initially. Something about new techniques of the allograft makes it better then in the past?

      @@jayjay80850 Honestly they normally recommend allograft for athletes over the age of 25 normally from what we have seen from orthopedic surgeons. The reason they have no so great outcomes because people often return to early since there is virtually almost no pain with allograft besides the first two days.

    Stay away from the hamstring graft. I had ACL surgery done 12 years ago and they harvested my gracilis and semitendinosis. I was never near the same athlete I was. Even walking has never been normal since. I was essentially robbed of my athletic life at the age of 22. I was naive and didn’t do any research and just went for the surgery. Make sure you do your research or it’ll cost you as it did me.

      Got the quad graft back on April 21st. Surgeon talked me out of using patellar tendon. Praying I make a full recovery to get back on the court!

      @@TheBasketballDoctors Thank you. I'm currently trying to receive stem cell treatment to help reinforce and regenerate tissue for the remaining compromised tendons I do have. As I live in Canada regenerative treatment here is limited compared to the US and Europe.
      2 weeks ago I was doing some moderate sprinting drills and my hamstring suddenly snapped resulting in a grade 2 hamstring strain with a 2cm by 1cm tear of the biceps femoris along with undiagnosed damage to the other tendons not observed under ultrasound (MRI imaging results here typically take several months to obtain). Despite a very clean long-term diet with rigorous weight training and strengthening throughout my life, the anatomical deficit I have just could not handle the activity load of a few simple sprints.
      Again it's quite disheartening and as I mentioned in my previous post, my life has not been the same since this surgery 12 years ago. This recent event has shifted my focus away from many things and I am focused now on finding solutions to improve my long-term function. If you have any insight on this or know of others who may please let me know. Thank you.

    I have surgery at the end of the month. Tore my ACL playing basketball sadly. I would say I'm a pretty active person and do love playing basketball at the pace of a shifty guard. I jump around a lot so I'm pretty active in terms of driving in the paint. So I'm extremely unsure which graft to go with. My surgeon says either quad or patellar. But it's my choice. But if I leave it up to him then he's going to do quad because that's the one he does more often. He also says he believes the quad is going to be what most professional athletes resort to in the nearby future. I am a 5'6 170 lb just turned 22 male. Looking for any advice :(((. I don't want to end up being that 40 year old uncle at the family cookouts with a bum knee lmao.

Research and treating clients quad and patellar tendon are pretty similar in terms of outcomes, but we have seen less knee pain with kneeling with the quad versus patellar tendon.
