there is a mistake in the video the definition for each is wrong it should be like this: Bug: A software bug may be defined as a coding error that causes an unexpected fault or flaw. Error: A mismatch between the program and its specification is an error in the program. Defect: Defect is the variance from a desired product attribute (it can be a wrong, missing or extra data). Failure: A defect that causes an error in operation or negatively impacts a user/ customer.
The video uses the term "Issue" to define failure, but does not define the term "issue". What are expected results? Expected by whom? What if a person incorrectly interprets and documents a requirement, or incorrectly interprets a requirement into a design? I've had cases where I am told the software is functioning as designed (so too bad), to which I respond that the problem is with the design. The design was wrong. By the definitions here, this cannot be a defect, bug, or failure. It might be useful to enter traceability and conformance into the understanding of the terms. Requirement incorrectly interpreted - defect in the requirement. Design not conforming with a requirement - bug or defect in the design. Code not conforming with the design - bug or defect in the code. Product not conforming with the design - bug, defect, or failure of the product. Coding errors are errors, some of which can lead to bugs, defects, and failures. I'm not a big fan of different terms.

A bug is a result of coding error. A defect is deviation from requirement.
