So you just use information on the birth certificate. There are always two names on all the birth certificates. So the person's third name is automatically taken from the father or mother. There's nothing to worry about.

I wish you were legally married. It will be trick for your case since you don't have the marriage certificate. If you can not reconcile and process the certificate, just don't proceed.

You did a mistake by not adding her. You can not add her on the ds260. you will be refused visa if it is known that you did not include her in the application

What about different place of residence in passport with the ones for your derevatives...is there any effect during interview

Should not be an issue at all, however it is good to arm yourself with affidavit incase you meet an inquisitive consular
