I have a Pi4 in a flirc case. It has been running constantly for about 18 months as part of the fold for covid project. It has only ever been warm to the touch.

The dots on the thermal pad puzzled me when I got my case too. Undoubtedly, many people have pondered that question and will in the future. I still don't know if it matters which side goes where.
Unfortunately, there is no access hole for SD memory card, but aluminum casting case is good.
I made every installation error, too.
I have a zero w (1 not 2) and i wanted to buy official case i think it will be enough cooling even with lid closed but do you have a temp test of it in official case also its a git server it will never run as hot as stress test
And how performed a cheap little heatsink without a case?
i find it so charming how you laugh about yourself, i would have done it exactly the same way ;)
