Can a Depressed Person Have Good Days? - Atypical Depression

  • Usually we think of someone who is moderately to severely depressed as being very sad and unresponsive to things. Maybe even zombie like. But that isn’t always how it looks. But when it does look like this, we call it having melancholic features or a melancholic depression.
    When someone is able to have good days or still enjoy some things, we call it having atypical features or an atypical depression. Atypical depression is the most common way depression looks.
    Here’s a comparison of the two types:
    Atypical
    Mood lifts
    More appetite
    Leaden paralysis
    Rejection sensitive
    Worse in afternoon
    Melancholic
    Always sad
    Less appetite
    Slowed or agitated
    Guilt focused
    Worse in morning
    Atypical depression has been shown to respond better to a different kind of antidepressant called monoamine oxidase inhibitors or MAOI’s. These drugs aren’t used as much as they once were 20-30 years ago before the selective serotonin reuptake inhibitors or SSRI’s came out. Those would be medications like Prozac, Zoloft and Lexapro. These medications have less side effects overall, so they are better tolerated. But they are not as good for atypical depression.
    The MAOI’s fell out of favor because they have a lot of drug interactions and a serious interaction with foods high in tyramine. If you consume those foods while taking one of these drugs you could have a hypertensive crisis where your blood pressure shoots up very high. And This could be fatal. So people taking the medication were given a list of foods that you couldn’t eat while you were on the medicine. Some of these foods were aged cheeses and wine. You also have to be very careful taking certain prescription and non-prescription medications including some allergy medications. Those interactions could cause serotonin syndrome which could also make you very sick to where you need to go to the hospital.

      I have kind of an odd question... Im a 23 yr old who has BPD (not that this matters in this question). I used to baby sit two young (3 &4 yr olds) boys, when the 3 yr old would either: say he has to use the restroom, then stands, stare at me and pees on himself (which to me felt on purpose but hes 3) OR dump toys out all over. Then he'd see me get visibly upset, id say (sometimes yell) something like "why, now help me clean this up"? He'd drop to the floor automatically like a rag doll and began to cry screaming "I can't... I cant move my legs/arms". Id try to pick him up but eventually id have to leave him and he'd be fine and moving minutes later. So finally the question is was he experiencing some type of "leaden paralysis"??? Or something else??

      Cheers for this, been searching for "how to remove depression from mind" for a while now, and I think this has helped. You ever tried - Hanincoln Nanlivia Framework - (just google it ) ? Ive heard some interesting things about it and my brother in law got amazing results with it.

      I have more symptoms associated with the "atypical" feature, but a couple of the "melancholic" ones actually fit me better.

      Yeah, I fit in the atypical, except of feelling better in the morning, for me it's the worse in the morning on the days it hits, and almost every time it happens I also get euforic in the evening, my entire percepton gets shifted, but I gues it's because of the hpd

    This was very helpful, thank you! My depression is certainly "atypical" and it helps to know my depression isn't "wrong" or illegitimate just because it doesn't match the symptoms of melancholic depression.

    I definitely have a lot of symptoms of atypical depression, and i always feels like I can't talk about it with anyone because it feel like I'm being dramatic. Thank you for making this video

      Same. I reached out to teacher about it years ago but he said it’ll pass. It’s been two years and I’ve been waiting to feel happier even tho my outlook and life style has kinda changed the feeling in me have still lingered. And I’m unsure of what to do, since no one takes me seriously.

      Honestly, self diagnosis isn't as bad as people make it out to be -- do proper research first and seek treatment. It's important to recognize that some people simply can't afford it either. Just be cautious with self diagnosing.

    I’ve struggled with Depression since I was a child. I feel like I teeter back and forth between the two types sometimes. This really helps me understand my disorder though, thank you!

      I definitely go back and forth as well, though this video was helpful to see how many of my symptoms that present at the same time tend to fall into either one category or the other.

      Yeah same here. When I am Happy in happy moments, I try to convince myself that I am alright, I don't have depression or I was just faking it. But then again after some time, Depression kicks in so hard, life becomes shit again.

      @@IISuperw If you have access to a therapist or doctor you should really see one. Even being in therapy for a month or so can give you the tools to find and maintain a better balance. It's helping me.

    I am bipolar and my depression always confuses me because some episodes I am sleeping in, always tired, eat more - then some episodes I can’t sleep, eat, and I am super anxious...this kind of episode confuses me because some times I rely on my sleep patterns to know if I am manic. This breakdown of atypical and melancholy depression really helps figure out my disorder just a little bit better. I think this will also help me better understand if I am having a mixed episode, or if I am just experiencing melancholy depression. Your videos have been very helpful for the last year and a half since I was diagnosed. Thank you for the upload.

    Thank you so much!. I low key thought that I was crazy for thinking that I was depressed because I’m able to go to school and kind of socialize. I’m an African and depression is something that isn’t really acknowledged or talked about as it should be. Thank you so much

    Leaden paralysis - for me it is SO severe that I'm often suicidal in the mornings. It feels like I can never ever get out of bed, thus never ever earn money, ask for help, get up to eat/shower, it brings me to this very lonely very very dark place, with an awake mind but a body unable to respond. Why is this not being taken seriously being medical professionals? I've mentioned this so often, begging for help. It will one day be my killer, maybe then they'll understand how severely one's life is affected by something so 'simple'.

      Hi Vicky thecat... I have this same type of depression!!! Oh my gosh, I didn't think anyone else suffered from the same thing. I've been researching types of depression forever and have never heard any describe it exactly the way I do. 😊😊😊 I used to wake up and be completely unable to move, even if I was uncomfortable or in pain, I just laid there for hours. I was suicidal, planning how to do it, and actively getting rid of things so no one would have to clean up after me, but even when I was so depressed all I could do was lay there. The longest I laid in bed was 23-24 hours without getting up to even use the bathroom. The longest I haven't showered was 21 days, didn't brush my teeth for 11 days, didn't cut my hair for a year, had to chop it all off bc it was getting tangled bc I never had the energy/motivation to brush it. I barely ate anything, I lost a lot of weight. I'm only a few pounds away from being underweight. I only ate 300-700 calories a day, sometimes I didn't eat for 5 days at a time. I haven't been able to work for 1.5 years, and my work history prior to that was very spotty. I was working about 14 hours a week. I felt/feel like no one would understand how bad it was, that they would think I was just being lazy or making it up for attention. I thought they would just tell me to get a job and I won't be depressed (doesn't work like that bc I've worked full time and overtime and was still depressed lol).
      It turns out I am an INFJ (rare personality), I have Avoidant PD (fear of abandonment and judgement), OCPD (not OCD!!), anorexia, cotard delusions (tho that has gotten better since I've learned about GI health), crazy bad depression that meets the criteria for several different types including PMDD. I highly recommend that you check out Frank James' old videos (he's an INFJ), ck2d (the only AvPD source I've been able to find), ocpd: my life in debris (he has OCPD not OCD), Dr. Todd Grande's videos on personality disorders (not the murder mystery type videos he does), and of course Dr. Tracey Marks' videos! She made a video called "Brain Zaps and Antidepressants - Why Do they Happen?
      " that inspired me to get on a low dose of Prozac. I had tried Citalopram years ago and hated it. I felt like a zombie. Prozac has helped tremendously, as well as learning about psychology and different types of personalities (MBTI). You can't change who you are, you can't change the world, but there is something remarkably helpful about understanding that it's NOT YOUR FAULT. Also, my mother turns out to be a communal narcissist, which explains a lot. That was hard to accept. Patrick Teahan LICSW makes videos about recovery from Narc parents (they are not narc bashing). The type of person I'm most likely to fall in love with is Histrionic (pathological cheater) and/or Narcissists. Idk if there is anyone that I'd feel comfortable spending my life with, but being alone is far better than being with toxic people. I'm working on building a life that I will be happy to live, alone.

It is only people who suffer the same way as we do that probably can understand the severity of it all. I'm the least lazy person I know - always top of my class, always a workaholic, but just like you, my cv has many holes in them. What were they? Me unable to move in the morning. My limbs not working, my brain not able to send the signals to say 'ok, get up now, you stink/you need to eat/drink'. I even peed many times in my bed while fully awake because I could not get up. I will look into everything you wrote about. Thanks again; let's stay strong together, fight our demons, fight them for future generations, raise awareness. Wish you all the best in life.

      This used to happen to me. Trust me when I say this. I know how you feel. Im better now and have very few episodes because I keep praying to God. If you want I will pray for you as long as I have permission.

      @@vickythecat1741 I should also mention that I split my Prozac tablets, I take half in the am and half in the evening. The first week or two I experienced some hypomania (insomnia, irritability, talking to myself lol), but I was also exposed to a very stressful life event the same day I started taking it, and the trigger came back a few times after that. The hypomania wasn't anywhere near as bad as what people with full blown mania experience, so I stayed on, and like I said, it faded. Splitting the pill might help you if you experience that, but be careful not to overdose if you decide to switch your pill schedule (don't take a full pill at night, then half a pill in the am, bc you'll be over.. take half a pill or skip a day after you've taken it for awhile).
      Also btw, I just got some blood results back... I have very low white blood cells, low platelets (maybe a clotting disorder, hopefully not cancer..), and low TSH (hyperthyroidism, perhaps Addison's). As I said earlier I haven't been able to work for 1.5 years. I have also had a ton of medical problems that were never diagnosed despite seeing multiple specialists years ago. For the past few years I have felt that I had an autoimmune disease, bc my energy level has been so low. My new hospital is so great.. they have an online system where I can see my test results as soon as they are posted, and they list what the normal ranges are so I can see where I stand. Very easy to understand. As I said my mood is so much better, but my energy level is very, very low. Perhaps you should consider getting some blood tests done as well? You might have some type of adrenal/cortisol/thyroid fatigue syndrome that is either causing or contributing to your depression.. It's never simple, is it? I hope you're able to figure it all out soon!! Best wishes 💜💜💜

I'm glad to hear you are doing better! Sometimes I can feel one coming on, I've even cried a couple of days, but it was NOWHERE near as bad as it was before. I can move. I can get out of bed. I am able to shake off the sadness and find something else to do. I never thought I'd be able to do that. I hope you continue to do well 💜💜💜

    My first trip to a mental health professional was when I was 10. I'm 60 now.
    That long ago they really didn't have a handle on what I was suffering and my parents were told "I'd grow out of it". I never did and the depression was bad, fast forward and my first daughter passed away. It was like the worst gut punch you can imagine. 2 years later my wife left with my next daughter who I've never known. I've been homeless 4 times, never had much of anything to my name. I've been a very mean person and found out that people who hang around people like me were mean people.
    Now that I'm 60 and on medication I am living with my mom because her and I are the last ones. When she's gone it'll be just me. I'll make it, I always do, just have lived a sad life....

    I have chronic depression, I’ve been diagnosed with it by 3 different therapists, and I definitely have those atypical depression symptoms that you named. Thankfully SSRIs and therapy has helped me deal with it better.

    It's been such a blessing finding your work, your videos have helped immensely with assessing myself - it's equipped me with a bit of a head for what to tell my family doctor to explain what I'm going through to get the help I need to work through the problems even my symptoms get worse, I just seem to find another video of yours with precisely what I need to know.

    Thank you for this clear, no -frills, informative , video. My mood lifted just from coming to understand what sort of depression I experience, and knowing that it's possible to have features of both melancholic and atypical. "Leaden paralysis" answered the question as to why I sometimes feel like there are weights all over my body. After seeing this, I'm now considering that the "fibromyalgia" I'm about to be seen for, may instead be depression, or that depression may be co-occuring.

    Thank you for the informative , helpful video. I have major depressive disorder and I get confused because sometimes I laugh one day at something on tv or in real life but the depression doesn't leave. It's hard to explain because people/family expect me to act suicidal, but when I feel ok one day, they think I'm not depressed. It's like "you don't look depressed" so you must not be depressed. Many people don't realize that a person can be depressed and suicidal and hide it.

    Hi Dr. Marks, New subscriber here! I’m enjoying your videos very much, and like the format of useful information that isn’t drawn out into a lengthy explanation. It captures my interest better. I really liked the explanation differentiating Atypical vs. Melancholic depression. It clarified a lot for me. I’ve been diagnosed BiPolar2 Depressed for many years, well maintained on Effexor and Lamictal, but the descriptions you gave, describe me perfectly on any given day, having features of both types. I guess what I’m trying to say is that it gives me some degree of consolation knowing there is a name and an explanation for how I feel, and i will no longer feel the need to justify it when I have the good days or the not so good days (I don’t like to assign “bad” to what I see as normal for me). Thanks again. I look forward to more of your videos.

    I always thought something was up with me but couldn't exactly say I was depressed but this video really pinned it down...I wake up at noon a lot, like 4pm and 6pm even, I have so little energy every day to the point where people ask me if I have even eaten anything(I eat fairly healthy), and I'm overly sensitive to what others are signaling in interactions. My head is generally always in a funk, and I thought everyone was like this in their adulthood, but then found out that some people actually spring out of bed in the morning and get going.

    I've always felt guilty for having good moments... It makes me feel like an impostor.. And I am desperately concerned with being authentic. I use humor as a coping mechanism, and i don't look depressed when i'm laughing at a funny joke. This has been so hard to deal with.

    I read in a book once about a character who “didn’t ever live life well.”. I instantly related to that description. I think my father was like this and I see parts of this in my children. It breaks my heart because in general we are very happy people! The atypical is so spot on... most nights I can’t bare to close my eyes...it’s like having this heavy blanket on and off my body...when I’m free of it, I feel like a million bucks!

    Dr. Tracy Marks, thank you 🙏 for making all these video’s, they are so good and full of info. I have dysthymia disorder and also get depressions with it. Double depressions. But this video was also very helpful. You make things very clear and explain a lot about all the other mental health diseases. There are so many and it’s so important that people get the right diagnosis. ❤️

    Wow! First time I've ever heard someone actually acknowledge atypical depression! I totally fit the description with the only exception being the depression is usually worse in the morning. Are any of the MAOIs more "mild"? The SSRIs have only been effective in the short term and have a particular side effect that really impacts my quality of life. Thanks for any direction you can give!

    It must be possible to have a mixture of both because I'm on both sides of the spectrum. I have just found a channel that will finally give me a better quality of life. I do not want to take meds. Thank you Dr. Marks! As you spoke of in another video I will try a path of mindfulness. I also realize that certain types of food, obesity and lack of exercise can contribute to my problems. That being said it's hard as hell for me right now to string 3 good days together. When I do I'll let y'all know.

    Thanks for sharing this video. You seem to have a lot of room for subtlety in your mind for issues such as depression, which is refreshing (and rare). I don't know if you agree or not, but for me the atypical depression feels more like a symptom of something else that also deserves attention. I am personally thinking of CPTSD. I'd love to hear your thoughts on whether different types of depression indicate a primary vs. secondary area of treatment.

      Hi Barry thanks for your question. Depression can certainly come along with other problems like personality disorders and even medical problems. Depression can be a manifestation of gluten intolerance Or a parrot after a heart attack or stroke. But we would still treat it equally with the original disorder if the symptoms were severe enough. In other words we wouldn’t say let’s address your CPTSD first and then your depression shit resolved. We will retreat the depression simultaneously with the other problem.

      I seem to have features of both as well. I know that I definitely have Avoidant PD, which she said would most likely mean I have atypical depression, but I have 5/6 Melancholic features, and only 3/6 Atypical features.
      You should check out the channel Polar Warriors, he has made a lot of videos relating to Bipolar mania, and other types of mania and depression. I'm not bipolar, but I find his videos very informative, relaxing, and helpful. 💜💜💜 Good luck

      Hi Lorelei. Yes you can have both it doesn’t have to be a clean separation between the two kinds of symptoms. With mixed mania you have a combination of manic symptoms and depression symptoms.       That's crazy. The Dr answered this question. That's wonderful. I was also interested in this same dilemma. I also believe I am in very deep situational depression if there is such a thing. I'm 56 and my life has completely fallen apart these last three years. It resembles ZERO of what it was before. It's hard finding any footing.

    Hi Dr. Marks, great information! I was diagnosed with Recurrent Brief Depression which seems to be a form of rapid cycling bipolar II. I go from 2-4 depressed days to 2-3 hypomanic days over and over. I really have to plan my life so the hypomanic days correspond with an activity or work needing to get done. It has been hard to stay in a job and relationships. I have not found much information on RBD so it may be a good topic for one of your videos. Take care!

    This Dr is great. This channel is great. So easy to understand, and a joy to listen to. I wish she were my doctor, haha. I'm going to find one, mustering up the energy to (do much anything) is a serious challenge currently. I need help but I've lost the drive and courage to help myself. I think I would be fine if I knew my rent, utilities and cable would be paid, and a small stipend for food, I'd be OK lingering away in my apartment. It's not ideal...
    My problems seem such a tedious story, action seems either daunting or lacking merit. IDK... I haven't given up yet and think getting help is key,. I'm convinced talk therapy is mandated, as I'm almost certain I'm experiencing crushing situational depression. Hence I worry about a just Rx or a "quick to throw" medication (solution) to the problem. I'm not adverse to meds but I've been prescribed this as an answer already.
    I'm 56 and there is nothing in my life, nothing, that is the same as it was three years ago. It took some time, but once the ground gave all footholds continued to crumble. Ugh... I go on with the laborious drivel. I haven't given up on trying to find or create a way out. I have recently stopped drinking. I believe this has to help, if that's the only thing that hasn't changed in my life then let's add that (in the plus file) to this inevitable change I'm own.     THIS WAS SO HELPFUL!! Thank you 💜💜💜 I definitely have AvPD and OCPD, but I have 3/6 Atypical features and 5/6 Melancholic features. I also meet the criteria for PMDD. I started taking Prozac bc of a video you posted called "Brain Zaps and Antidepressants - Why Do they Happen?
    " which explained why Prozac is sometimes more effective than other SSRIs. It has helped tremendously! I think I speak for everyone when I say that figuring out what kind of depression you have is very confusing. It's confusing even when you're emotionally stable, but when you're dysphoric, nothing seems to explain how bad you feel.
    Your videos are SO helpful. You are an amazing human being!! 😊😊😊 Would you be able to make a whole video dedicated to explaining all of the different types of depression? I'm not sure if there are too many to do this. Do you know if certain types of depression are more common in certain personality types? For example, I know ENTPs are most likely to have HPD, Bipolar, Major Depressive Disorder, OCD, and that HPD can often comorbid with other Cluster B PDs. I am an INFJ, AvPD, OCPD, anorexic, have some type of depression. I often have feelings of Disgust, Fear, and Sadness. It is amazing how everything correlates to each type.. Psychology gives you all the answers to life!! 💜💜💜 Btw, I think you are an ENFP... my favorite type of people! I've had ENFP best friends my whole life. I love your personality, you just radiate creative energy! You're so spunky and unique. Never change! 💜💜💜

      @Al Za I don't take any offense to that at all. You're entitled to your own opinion about things. However, personality type is not even close to the same thing as Zodiac signs. MBTI correlates to the FFM (OCEAN) that psychologists/psychiatrists use to evaluate patients. Zodiac is not scientific in the slightest bit. I've met lots of people with my Zodiac sign but totally different personalities, yet the people with my Personality type all share remarkably similar thoughts and behaviors.

    I'm finally going to see a psychiatrist to hopefully get a diagnosis. I feel like I have depression but not really and it makes me feel invalidated. Sometimes I even think if I'm just acting out and all of this is just in my head. It's good to know that there are different kinds of depression, makes me feel that I'm not the only one feeling like this.

    I definitely have atypical depression with PTSD and BPD features. I was on Nardil and Parnate for years and it was helpful. I was fine with all the interactions and the dietary restrictions. I actually would like to switch from taking Effexor and either go back to an older MAOI or try one of the newer ones. I will discuss this with my doctor. I am so grateful I found this channel..... but one that's sticks out to me about melancholic depression is being up shit from years ago and feeling bad about it and wish they could change it me 😭 and I was diagnosed with bpd and I sleep a lot even when I sleep I am still tried I am always tried

    A lot of the symptoms in atypical depression fits me to the "T"! I also have been diagnosed with schizophrenia. The drugs that you described sound interesting, maybe the MOI's, but I was wondering if there is something that I could add diet-wise or omit diet-wise that will help with mood? Thank you Dr. Marks!     I now know that I have major depressive disorder with atypical features, and generalized anxiety.
    An ideal party with friends would be something I'd like to engage in, and it would make me happy while I was there. However, when the party was over, the happiness drained out of me within an hour; returning me to the default low affect state.
    Complicating this further, it would take significant emotional and mental energy to be at a party, even one I enjoyed. I would need long recovery periods after experiencing the things that made me happy.

    Could you talk about kleptomania, it's treatments, and how it relates to other illnesses since I hear that it most often is comorbid with something else? I am bipolar, am going to be tested for adhd soon, and have kleptomania that peaks when I am hypomanic/manic, and my mood stabilizer/antipsychotic that help control my bipolar have helped a lot with controlling the impulses. Not removed them completely, but made them more manageable. I don't hear very much about kleptomania, it's something I am ashamed of having but would still love to hear it talked about more!

    I hate over thinking things but I do it so often, I was crying late at night and knew my friend was up and so I texted her and I was typing out that i was crying and needed someone to talk to and then she changed the subject and Asked me if I wanted to see something she made and so i had delete my message and just say sure. After that she asked what I needed her for and I didn't want to make her sad after she was happy I just said nevermind and just went to RUclips trying to find something to cheer me up and by the time I was finally able to go to bed it was 2 Am. It also sucks the fact that I wrote all this out and I just want to delete it and if I don't post it now I'm just gonna click discard

    I too am both types...
    Atypical
    - I'm able to lift my mood with things I really enjoy, but...
    Melancholic
    - I find hardly anything interests me so it's difficult to engage in and thus enjoy things.
    Melancholic
    - Chronic appetite loss. In the day, I literally feel like I have no stomach.
    Atypical
    - I sleep a LONG time and am SO sleepy, mostly from 6am until 1pm, but I go to bed at 2am I've always been a night owl, which brings me to...
    Melancholic
    - I take half an antihistamine every night to sleep and before I did this, I would be wide awake until 2am, but instead of sleeping properly after that, I would wake up very easily and extremely early, and be wide awake, feel wired and very tense. This has been since childhood.
    I've thankfully never experienced leaden paralysis.
    Melancholic
    - I do have major guilt. I feel I've done a lot of things and I feel a sense of hopeless panic if I dwell on them. Hence, I need the TV on all the time, avoid being anywhere without distractions (for instance, the shower or laying in bed not sleeping), and if I must be alone with my thoughts, I go into what I call "daydream mode" where I fantasise about all sorts of things I'd like to happen, have conversations in my head with people I know "in real life" (if I'm alone, I will talk out loud during these imagined conversations).
    - I tend towards irritability, intolerance and impatience to, for example, slow walkers or people and things who annoy me, I get angry, feel restless and bored a lot.
    - I DEFINITELY feel worse in the morning. I think I sleep so late to escape having to feel anything, and to sometimes avoid the mornings entirely. I find I start to feel less hideous and more normal after about 4pm-5pm, and it improves as the night goes on, and I can relax (somewhat) and zone out with my reality TV shows.
    So maybe I lean more towards the Melancholic type.
