These guys are comparable in age to her but she’s calling them younger brother which would usually be an incorrect label. Normally they should be at least 5 years younger to be called that.

They're confused, because the relative age isn't very obvious. She doesn't look significantly older than them, or even necessarily look older than them. I don't think they feel demeaned or belittled, just wondering if they look young, or she's older than how she looks. If I'm not wrong, if two people look like they could be similar or close in age, female would greet male strangers as elder brother, male would greet female as younger sister.
