Jaw surgery Case # 104: Johan - Sleep apnea, maxillo-mandibular retrusion

  • Johan visited us from Sweden after his first jaw surgery failed to fix his sleep apnea and left him with a poor cosmetic result.
    In our center, Prof. Federico Hernandez Alfaro performed a revision surgery that consisted of three steps:
    1) removal of fixation material from previous surgery
    2) maxillomandibular advancement with counterclockwise rotation
    3) septoplasty
    In the video, Johan speaks to us 24 hours after his surgery, and tells us how he feels up to that moment.

    I would like to see the decrease in tooth show addressed. I don't see it in the VSP. Was any impaction done in the end? In the VSP it looks like there is even anterior downgrafting. Why the decreased show?

      I agree. I have seen excellent results from Dr Alfaro, so curious to understand what the tooth show is less here compared to other cases.

      In the after photo there is still considerable swelling in the upper lip area, therefore, lip-retracting muscles are still swollen and not functioning as they should. Toothshow will go back to normal after some time. Hopefully he will be back in Barcelona soon to be able to post an update.

    You have many good results.
    Do you put emphasize on lips sealed?
    because if the mouth remains open it will relapse.

    When the jaw bones are advanced or moved in general, how do they stay connected with the rest of the skull? implants?

      Hello, if you're orthodontically ready, right now we are programming surgeries for the begginings of 2023

      @@InstitutoMaxilofacialBarcelona Hi, in my case it is only a mentoplasty. I had an online consultation with prof. Alfaro some time ago. In general, is online consultation enough to go through a surgery or a patient needs to fly to you for an in person consultation (months before surgery)?

      @@mjaumjaumeowovich7166 Normally patients arrive 3-4 days before surgery to finish the planning, take models and do pre-op. Not necessary to come beforehand.

    Hello - I have a quick question. Does age matter when undergoing this surgery? How bad will healing be? Thanks.

      Hello Dan, there's no age limit to undergo this kind of surgery. Recovery, however, it's definitely going to take longer than in younger patients, as with every other surgery.

    Hello, does he experience any paralysis / nerve damage? For me the biggest concern is the risk of having permanent numb area on the face.

      Hello, no, he didn't experience any nerve damage. Numbness is a real possibility when having orthognathic surgery, but a well experienced surgeon can diminish that possibility to almost 0%

    How come you only post about successful surgeries? It would be interesting to learn about people with bad recovery times, numbness or cases in which they regret, no?

      That's a weird question. I wonder if you've ever seen a hairdresser, handy man or any other service provider post bad outcomes of their work. We post the people who want to be posted, there are also people with gorgeous results who refuse to have their faces on social media, so we can't post them either. Normally, if someone is not happy about their results, they don't want their faces posted for everybody to see.

      @@InstitutoMaxilofacialBarcelona I see. Well, I am considering a DJS, but I want to be extremely cautious and I feel it is very hard to properly learn about the benefits vs possible risks. I wonder what is the best way of doing that.

      @@ecm9134 We advise you to book appointments with several surgeons , nowadays it can be done remotely, and talk to them until you find someone with whom you feel at ease and overall, someone who has a similar idea of aesthetics as your own.

      @@ecm9134 In our case, we will ask you to provide some very specific photographs and a cranial scan, so the evaluation is as good as if you were here.

    Hello,
    i find the smile in the before better, you see more teeth, if he gets older u probobly will see no teeth anymore, but the manibular/chin improved well.

      He's still swollen, you can see in the simulation that his teeth did not move up.

      @@yourbrain8700 He's definitely not smiling like he was in the pre-op. There may have also been some upwards movement which either was not reflected in the plan or the maxilla relapsed upwards. He was a bit gummy before, it probably turned out alright.

      he probably needs a palatal expander and that should perfect his smile.
