My anaesthesiologist was pure magic, narrating her unique story on how she told her mum and dad she wanted to be an anaesthetist. Funny and profoundly moving, just before I went under. I just want to say I appreciate the fact that you guys are still reading and responding to comments. Such great care even from afar. I am due for a colonoscopy but keep putting it off due to the fear of anesthesia. I'm generally a very anxious and easily dizzy person so the idea of going under really holds me back from getting it done. Is regional anesthesia without sedation possible with a colonoscopy?

Great info, solved some interesting dinner table debates. How about a piece on sedation, since most of us (as you so rightly point out) are petrified of seeing/hearing/feeling/knowing what's going on. Having come around slightly during retinal surgery, it's bonkers! Don't want the same during the hip stuff.

As a retired oncology nurse I always tried to teach my patients that they needed to advocate for themselves, today’s medical treatments are done with such expediency, patients have little time to be truly informed. Thanks docs!! My mother almost died from the anaesthesia called Anetictine. When they got her recovered, the doctor told her to tell her kids to let the doctors know that this happened to her and they said to especially let me know. Did this happen from an alergic reaction to the medication?? My doctors have annotated my records with this and the anestheiest always asks me about this before my surgeries. I love watching you guys. I am learning so much! Thank you!
Hi Pamela. So this is succinylcholine. It is hard to say what happened because there are both dangerous side effects and possible allergic reactions. My first THR I had general-took days to recover from the fog. The second THR I had spinal- what a difference. I was so much more alert afterwards. Thanks again to three of the best Docs!
Thank you for another great video. I had anterior right hip replacement in April. I was told I couldn’t have a spinal because of my scoliosis and stenosis. The procedure went well, but recovery is taking a little longer than I expected. Nice not having hip joint pain.😊
Thanks Docs! Great discussion of anesthetic options. Many patients aren’t familiar with the fact that we only want to use the minimal amount of anesthetic to cover the objective. Over doing the anesthetic means greater risk for the patient and longer recovery time. Who WANTS that? Nobody. So. If your surgeon and anesthesiologist present you with the option for something other than a general anesthetic, listen up!!! A safe anesthetic is more than just a general anesthetic!
It might be useful to make clear exactly what "freezing medicine" is. This doc-speak terminology might suggest to some that really cold liquid is being injected or that it is some sort of cyrotherapy. It might help to give the medical name of one of the most common anesthetics used in orthopedic surgery.
I’ve had over a dozen surgeries and a variety of anesthesia. Two of the surgeries that I was supposed to be under a general I was awake enough to hear the surgeons making comments. At the first post-surgery appointments, I asked what they meant by “Oops” and “She’ll never notice that I had a problem with her nose.” They were surprised, to say the least. My worst experience with anesthesia was at the almost end of my total knee replacement. I say “almost end” because I was totally aware of being in the surgical suite and in immense pain before I was wheeled to the recovery room. It took an eternity of moaning, crying and sobbing in the recovery room before I was given something for the extreme pain-after the nurse said “You’re not the only one in here who’s in pain.” I still have nightmares about that. It was almost 9 years ago. Unfortunately, that replacement has never stopped causing me pain. What the he// happened? Am I that odd or did I get poor anesthesiologists for those surgeries I mentioned above? I would really like to know if there’s some way of testing the efficacy of general anesthesia BEFORE surgery.

I was able to here all the conversations in the room, I said to the surgeon, after he was telling the nurse his plans for going to the cottage and his drink recipe, “I am hearing all you are saying”! They then put me under general. I was sick and foggy for months! Hopefully no more surgery’s!

Love your channel x I had my first knee replacement, left, last August. My anaesthetist started to say about spinal epidural & I said straight away 'nope I want a full knockout job! I don't want to hear, see, smell anything!' Plus I mentioned my hubby in 2016 had a spinal epidural as part of his prep for a liver Resection. The needle didn't go in quite right & had to be taken out & reinserted. Hubby has a lump from the first needle since which sometimes becomes inflamed & painful. The anaesthetist was fine. Fingers crossed I have my right knee being replaced, tho later than planned, this Aug. I'll be asking for a General again!!!!!

Very informative thank you ! I recently had trigger thumb release surgery. I had two pulmonary embolism in January and respiratory failure. I was very concerned about full sedation so we discussed a bier block. My anesthesiologist felt general anesthesia was best. I'm not the nervous type and a retired nurse. Agreed to the general then developed pneumonia just after surgery. Ick !! Next time I'm going to insist on a nerve block if at all possible. My lungs can't take much more.
2 weeks ago I fell, resulting in a compression fracture of the L distal radius and a chip off the old ulnar stylus. ORIF plate & screws for the radius. Apparently a very common fracture. I opted for the regional block with sedation. What hurts/is most bothersome now (12 days post-op) is around my elbow. I only landed on my hands, the rest of my arm was not involved. Is this from the tourniquet during the operation to control blood loss? I am wondering if you could address tourniquet pain post-operatively. Thank you for all the informative and enjoyable videos you have posted.
Thanks for sharing and best of luck with recovery. Yes it could be from the tourniquet but also can be from the injury itself and some of the force does travel up your arm to your elbow. If it persists tell your surgeon.

I was wondering if you guys have any root causes of my situation, I had both hips replaced about three months apart. However, in each surgery I had to stay in the hospital for 3 days due fainting, low blood pressure, when I would stand up. Moreover, I did improve and was totally ok after 4 days. I never really got reason of this condition, other than for some people said, it takes more time to recover from sedation?

Hi Lokesh N! Glad you are liking the channel. So hard questions. We believe that yes pain can cause permanent trauma - like PTSD. That is certain. As for whether or not death rates would go up or down if you did not use anesthesia, we believe the answer would be that they would go up. If you have severe pain, that increases blood pressure and heart rate and would likely lead to more heart attacks and strokes. Hope that helps. Can you tell me why you can have a flexible cystoscopy with only numbing gel inserted in your urethra but for a rigid cystoscope being only slightly larger diameter patients require either a spinal or general anaesthetic? I’ve had a flexible one done which was easy and found a 2cm stone in my bladder and am on a waiting list to have it removed. 100% do not want a spinal due to risks of back pain, migraine headaches and urine retention and not keen on a general either so want to know what other options I have for this procedure.
Mostly due to the pain and the likely need to do more work during the same procedure. We use spinals 99 percent of the time for our joint replacements and it is a very safe and effective method. Headache is very uncommon but can happen. And if they use low dose or no narcotic urinary retention is not really an issue
What can you say about Ketamine. People are saying causes people over 70 to go into dementia. Is that true and how often is it used. I’m having knee arthroscopic surgery in March 2022.
Late to the party! Damn shiftwork. Interesting, never been offered anything but general anesthesia. Maybe because it was just day surgeries. Who knows. Does OTMH do genicular nerve ablation? My understanding is it's fairly new. Sunnybrook does it.

What about MAC anesthia?
I was wondering if you guys have any root causes of my situation, I had both hips replaced about three months apart. However, in each surgery I had to stay in the hospital for 3 days due fainting, low blood pressure, when I would stand up. Moreover, I did improve and was totally ok after 4 days. I never really got reason of this condition, other than for some people said, it takes more time to recover from sedation?
Hi greez dad. So it can be related to the narcotics which can lower your blood pressure and make you feel dizzy and faint. That plus the blood loss from surgery would be the two most common causes. Hope that helps. Glad you are ok. Thanks for watching and subscribing if you did
How do I convince my care team to do my procedures with general anesthesia when they say "its not the standard of care".
I'm still wondering why it was necessary for general anesthesia for my left inguinal hernia repair. It seems to me (a layperson) that an epidural would suffice. Water under the bridge now, but this video has re-stoked my curiosity.

Hi Lokesh N! Glad you are liking the channel. So hard questions. We believe that yes pain can cause permanent trauma - like PTSD. That is certain. As for whether or not death rates would go up or down if you did not use anesthesia, we believe the answer would be that they would go up. If you have severe pain, that increases blood pressure and heart rate and would likely lead to more heart attacks and strokes. Hope that helps. Can you tell me why you can have a flexible cystoscopy with only numbing gel inserted in your urethra but for a rigid cystoscope being only slightly larger diameter patients require either a spinal or general anaesthetic?
I’ve had a flexible one done which was easy and found a 2cm stone in my bladder and am on a waiting list to have it removed. 100% do not want a spinal due to risks of back pain, migraine headaches and urine retention and not keen on a general either so want to know what other options I have for this procedure.
Mostly due to the pain and the likely need to do more work during the same procedure. We use spinals 99 percent of the time for our joint replacements and it is a very safe and effective method. Headache is very uncommon but can happen. And if they use low dose or no narcotic urinary retention is not really an issue

Late to the party! Damn shiftwork. Interesting, never been offered anything but general anesthesia. Maybe because it was just day surgeries. Who knows. Does OTMH do genicular nerve ablation? My understanding is it's fairly new. Sunnybrook does it.
