This Advanced level "ChalkTalk" shows you how to determine the cause of a pause in the rhythm. Not all arrhythmias look like the textbook, and this is a great example of how even experienced ECG readers might make the wrong diagnosis. Learn how to correctly analyze arrhythmias that most people get wrong.
Dr. Nicholas Tullo, a heart rhythm specialist, presents this "ChalkTalk" for the ECG Academy. This instructional video features a change in the rhythm and is designed for people who already know the basics but want to get used to reading more complicated tracings.

You are right. I do apologize. These RUclips chalk talks are going to be running long-term. I am in the process of developing the videos for the website. So it's a work in progress. If you like my teaching style, you'll love the website -- I hope to have it completed by the end of the year. Thanks for your interest!

@striae -- Thanks for your comment. Generally, the AV node exhibits a normal refractory period that can vary with autonomic tone. Calcium levels are generally not a factor.

@jumpthehoops -- Non-conducted (or "blocked") atrial couplet. You won't find that in any basic ECG interpretation books. It's a physiologic phenomenon that you don't see too often, but the point of these ChalkTalks are to introduce advanced physiologic concepts that help you read difficult tracings -- these arrhythmias WON'T look like the ones in the book. I hope that helps....

Very simple question from a beginner - if we were to ignore the non-conducted PAC; how do we differentiate the ECG as a Mobitz type 2, or sinus exit block - I understand that Mobitz type 2 are usually associated with abnormal QRS, and vice versa for sinus exit block, anything else?

Dear doc Nick, could you tell us what kind of app you use to edit the ECG? I usually use MS paint 😅 with adjustable hollow rectangle, it works the same but I'd love to use that digital compass like the one in your videos. Thanks
