    What a spectacular series! To be honest I personally like the look of her dimples and angular bone structure that result from minimal buccal fat pads. That's just my personal opinion. Regardless, I would love to see more videos like this! Your techniques and tips are fascinating. Thank you!

    What gauge and what length of cannula did you use? Awesome job! I love how you filled in the hollows of the lower face. I also think she would have benefited from some bi-directional cog threads as well bilaterally (I know this was a filler based demonstration ) and maybe a bit of lip filler. This was a natural restoration that easily took 10 years off her. I’m sure she was so excited to see the results !
    I love this better than your other videos that just talk about risks and techniques. I am a visual person so please do more videos like this in the future !!!

      We used mainly a 50mm cannula 25G but I also like the 38mm too.

      With this much, she may have been. Most fillers have the option of having lidocaine in them so you are numbing while filling. Your patient really should only feel when you pierce the skin. Either way, keep injection points to a minimum and use a fanning out style from one location to treat multiple layers. When I do my own filler or threads, I don’t use lidocaine. There’s no pain after you are in the skin so there’s really no need. As a new injector, I’d advise you use lidocaine on patients to make you both more comfortable, but you will find yourself using less and less depending upon the patient. Some patients are just jumpy by nature. You’ll get a feel for it with experience and as you develop patient relationships.

    Beautiful. Question regarding use of cannula and aspiration. I did not notice you regularly aspirating. Is it because of cannula length or because you felt comfortable in that layer ??? Please elaborate if you would and thank you.

      I noticed that too. I think when using the canula in places he has already injected he can probably tell by the feel of the canula movement that he's actually in an area where he has already injected and is just filling it out. That was my guess.

    As a Novice I enjoy watching your videos. She has a significant underdeveloped deep lateral fat pads. I would have addressed this primary with a deep injection in the canine fossa and afterwards addressed the superficial fat-pads from the temple to the commissurs with a cannulae. Shadow chasing will, as I understand always give a bumpy look and take a lot of time.

    Can I use fat dissolve with this needle on my buccul fat? Where can I buy this needle

      This is something we wouldn’t recommend. Having seen palsy like effects after having fat dissolving done to the lower face close to nerves. The nerves have a special sheath that is made of fat and if you disrupt that equilibrium the nerve won’t work. It’s not a pretty picture.

    Is there any reason why you aren’t purely building up the jaw bone line rather than spreading filler into fat pads that will gravitate downward if and when they can?
    I had check filler placed with cannula that did migrate downward off the placement within a couple of months. The cannula use I understand safety wise but the more I see of ageing faces the more value I see in investing in bone structure filler rather than the changing fat pads

      Hi again Daniela and @Amy D . I spoke to Tim and he said that theoretically you're right that the filler could migrate/descend and cause that look. So, that's why product choice becomes very important. Tim uses the Juvederm Vycross range (Voluma in cheeks) and he's used it for so many years that he's confident that it integrates well largely without migrating. Thankfully this has been born out with Lindsay who still looks fab 13 months on. Hope that helps, Miranda

      @@anutherapie I had 1 Syringe Radiesse and love the results, key is use small amounts, then you will not have the problem in dissolving it! Furthermore, why dissolving fillers anyway? What is the point? Since you paid for it, and then dissolving?? Key is use small amounts to look natural, we can always come back to get more

      Hi. We use the Juvederm Vycross range. At that time they didn't have Volux available, so it was Voluma in chin, jawline, cheeks; Volbella in tear troughs; Volift in nose-to-mouth lines. Hope that helps

    I have a question: if applying filler along the male jawline (Radiesse) with a cannula, then when the person opens his mouth wide wouldn't the filler not stay along the jawline (as opposed to needle technique injected to the periosteum). In other words , wouldn't you see a 'line' of filler above the jawline when the mouth is opened wide? I am not an injector, but my understanding is that the cannula technique applies the filler more superficial. Is this correct? If so, I would think it wouldn't stay put when the face is moving, especially when opening jaw wide. Is this correct? Thank you.

    Great video! I was injected with filler in my cheeks 8 months ago for acne scars, and developed swelling in my cheeks, it swells up when I eat + looking way more swollen in the morning. My dr has been injecting high concentrstions of hyarulonidase and I first saw improvement after the 4th dissolvement session, but it has not totally resolved. What could have happened to me? Is the filler located in the salivary gland? Could it be an injury even tho hyarulonidase improved it? I had an MRI which showed increased fluid area aprox 10-12mm into my skin on the cheek where most my swelling is located, is it logical that that is filler? Thank you in advance, i of course do not expect you to comment but it would be wonderful, merry christmas from sweden

      It's possible there is some salivary gland injury - the gland may have a tear causing saliva to leak into an adjacent space, but it's very unlikely to be the same on both sides- is it symmetrical? Blockage of the duct is less likely and something a firm massage would hopefully resolve- this would also be one side only. If it is something you are getting on both sides an immune reaction is most likely and hyaluronidase should help get rid of it.

      @@DrTimPearce The majority of the swelling is on one side, where also most fillers are. But I definetly have swelling on both sides upon waking up and eating. If Hyarulonidase improved the condition, logically it would make sense to say there is no salivary gland injury?

      @@diskursen you need to go to your doctor and tell them you talked to a plastic surgeon who told you you may have a salivary gland injury and you need to be checked out and treated

      There's always a risk, and larger volume treatments more so- but I saw her 18 months later and there was no signs of that at all.
      I keep a close eye on larger volumes (my highest ever volume treatment 1as 30 mls over two sessions and it lasted extremely well- many years later there was benefit without any clinical signs of migration- but I do think it's right to consider this carefully.

      I suspect it's just a genetic variation rather than something the patient could control. I believe she was around 40. We are based in Manchester, UK.

    My questions you took very great care Here to restore the beauty as you can see and were clearly stating throughout and restoring the fat pads is this the aim of every professional I mean I believe also you were refering to this woman paid $4000 to have everything restored in one go I am concerned that if I get mine done bit by bit that the person who's doing the treatment may not be extremely well trained in the facial anatomy and have the same goals they may just stick a needle in to get rid of a fold and make it dissapear rather than actual precise placement to create the beauty I am after if that makes sense ??... I'm making lots of notes and educating myself I don't want a half hearted treatment for big Bucks does that make sense by time I have my treatment I will know exactly everything aboute everything from the experts and will I be able to go in and guide them what to do also I mean express what I want to a degree or will I just have to accept what they give me ??...hope you understand what I'm saying or where I'm coming from I don't want shoddy treatment I want treatment that gives the desired goals

      I m the same i see doctor luna in quebec canada and he goes little by little at a time that way i can aford it and he is magical to,and you have to trust your doctor if not dont do it ....and it is good to wacht videos like thisone because you will know what to inspect and by the way it was a incroyable video ......🔥⚡my advice start with what bother you the most that way you can aford it baby step 😁

    maybe we need to redefine the concept of aging and beauty - maybe we don't want to have a fat face after a certain age.

      I don't think we can redefine beauty- it is what it is.
      There is the beauty of the individual- which is more of poetic and timeless beauty- and then there is the biological beauty.
      Biological beauty In humans is largely focused on the female form in youth because it represents our most valuable asset as a species the limited time and potential to reproduce and create a healthy next generation.
      There is power in both, and it's up to each individual to decide what they need to optimise their life.

      One of the earlier signs of aging is loosing the volume that gives a tight young looking skin. I don't wish to age exposing my bones but smoothly cover them without deforming a beautiful natural face into a baloon. This woman is naturally beautiful and the achieved result is natural. What else can you ask for?

      @@DrTimPearce I don't know. I kind of disagree that beauty is all about youth and biology. Well, maybe chemistry. In the 90's it was the waif with the strong jaw. Now it's all about the curves. Years ago obese women were considered attractive. I think the idea of beauty for many people is defined by trends. I've had super hot boyfriends that became repulsive because of their personalities. I've also fallen in love with a poet/artist that I had turned down over and over. Eventually he won me over and I genuinely found him beautiful. Cookie cutter beauty is also just so boring. Don't get me wrong. I love aesthetics and minimally invasive procedures are wonderful. I just prefer that mine don't leave me looking like everyone else. I'm 50 and I love the hollows in my cheeks. It means I don't have to deal with time consuming contouring.

      @@maiselph575 So be yourself. If you don't want to be done on your body then don't do it. This woman desire to have a filler enhancement and let her be. The doctor just amazingly helped her to achieve her desire. Looking at the before and after pic, the doctor has done a great great job!!!

      Thanks for your comment Umm. Tim did a full consultation with Lindsay, and lips were not part of her wishes. It's important to only treat that which will contribute to a patient's well being, by their own definition.

    How does so much product in so many places look 5 months down the road? Doesn't filler move around and expand, essentially blurring all the delineation you so painstakingly worked on?
