Clinical tip: Midline Correction with Intermaxillary Elastics|【Chris Chang Ortho】CC451

  • Abstract
Asymmetric intermaxillary elastics are often indicated for midline correction. However, asymmetrical elastics in the posterior segment often result in occlusal canting due to unwanted molar extrusion. To avoid this side effect, a recommended alternative is to use z-shaped elastics in the anterior segment instead of asymmetrical elastics in the posterior for mild midline correction.

As a person who is in this procedure to correct midline shifting, it is good to know what my orthodontist is trying to do to correct my midline as I always want him to explain what he is doing and what process I’m in but he’s always in a rush. This helps me a lot. Good to know! Thanks a lot!

I think, the important moment under such influence is rectangular arcs. It is also necessary to take into account the final molar ratio. The arcs must be fixed to prevent slipping in the grooves. And I do not like clamping hooks.

Smart solution. Other possibility its use an elastik from 23 to 43... or a heavier elastic than the other side.

    hello dear Doctor
    hope you are doing great
I want to clarify some points if u don't mind dear doctor, 1. The long class 2 elastics in posterior segment + z-shaped will neutralize the effect of clockwise cant rotation ( the moment and force ration in long class 2 elastics produce big moments as u mentioned in the beginning)? why u didn't use only z-shape elastics?
    2. the z-shape elastic ( between upper central two opposite forces will equal 0 + on lower canine to premolar will it equal zero? then from 12 teeth distally to lower 33 teeth mesially will act like short class 2 but the force direction is to the upper center of Resistance so will have translation movement of maxilla but due to distance between force and center of resistance on the lower arch will still have some moment right?
thank you for your knowledge dear Doctor
hope to meet you one day and learn more from you

My orthodontics are so stupid. I have the last case and they put the elastics in the same directions (CII elastics), making the midline even worse. When I told them or recommended to put the elastic in differing directions (CII and CIII), he agreed with me. But now I am worried about my canted smile even becoming worse as I already have right upper teeth more intruded. It is so frustrating. Is it rude If I show them this video?

@@johnh5149 Obviously I missed your reply but listen Sherlock, I was complaining about the risk of occlusal canting as shown in the last case. I am not the orthodontist, they are so When I brought up that my midline is off and advised elastics in different directions to line up the midline, they SHOULD have known about occlusal canting and the elastics SHOULD have been exactly as the last case....NOT just class II and Class III. My upper teeth are concaved in because of that reason now.
Ps; after SIX years of braces, I learned the hard way that NOT everyone knows what they are doing and you have to speak up and express your concerns.

I had perfectly aligned set, but after completing the treatment ( to push the teeth back), I realize my teeth were no longer aligned. I consulted with the same doctor who did my treatment, about my concern. To this the doctor suggest that the only way to fix this is by pulling out one healthy tooth to have enough space to move teeth for aligning.
This video, show there is no need to pull out healthy tooth in order to do the aligning.

However, I keep trying to tell him that this has only moved my 1st molar, with no movement of the K9 tooth at all. It has not only pulled the molar in towards my tongue, but it also pulled it out some. To the point that I have now grinned down the tooth. @rienne89 2 месяца назад

Hello, I hope your okay,. I ask Kriss Ai about your concerned and it said Midline shifts and the creation of gaps can be a normal part of the orthodontic process. Treatment often involves moving teeth in stages, which might temporarily cause these changes. Trust the process, as these temporary changes are steps toward achieving the final, optimal result. If you have concerns, communicate with your orthodontist for clarification. Remember, the journey may have temporary inconveniences, but the end result is usually well worth it. I hope this help.😊😊
