  • In the above video, Dr Ravindra from Institute of Emergency Medical Sciences (IEMS, Indore) is demonstrating how to do emergency needle thoracostomy with seal for tension pneumothorax. Many thanks to him.
    The above is a temporary measure till the definitive management by ICD can be done.

    Great demo , one addition Now needle Decompression site has been changed to 5th ics ,10th ATLS guidelines since 2018
    For demo iv cannula is ok but in real life you need at least 7 cm long needle for decompression if you don't have just do finger thoracostomy
    It's not needle thoracostomy but needle Decompression or paracentesis
    There is no need to waste time to prepare for under water seal drainage as it's life threatening and 20 ml syringe is ok or do simple decompression followed by chest tube . Regards

    According to ATLS guidelines, it is now 5th ICS slightly anterior to midaxillary line

      in tension pneumothorax, if you follow the recommended site, chances are very less. above the rib is the most important thing to keep in mind as this avoids the vascular bundle running at the lower surface of the ribs.

    On a mechanical ventilator which will appear first
    A drop in blood pressure OR
    A drop in SpO2?
    I have missed tension pneumo multiple times due to this confusion

      Depends on the acuteness with which it develops ; it will fall in spo2 followed by fall in BP ; importantlyu don’t have to wait for all this to develop , clinically examine , decrease breath sounds and resonant percussion .
