1. Your doctor will need to write a prescription for a seating evaluation .
2. Your doctor will need to write a prescription for a manual wheelchair and a seating evaluation.
3. You will need to schedule an appointment with a physical or occupational therapist along with your ATP to do either a virtual assessment or an in clinic visit
4. the ATP will bring out a demo piece or pieces of equipment for you to try he or she may take anatomical measurements at your seating evaluation or when he drops the chair off at the demo.
5. The in-home portion of assessment is performed by the ATP.
6. The demo. For pieces of equipment generally last 2 weeks.
7. The ATP will fill out order forms mapping out the new wheelchair for each person.
8. The the physical or occupational therapist will fill out the hashtag letter of medical necessity or LMN this is a basic form that justifies to the insurance every cent of the cost of the piece of the equipment
9. After the letter of medical necessity is written it gets submitted to your doctor for her to read it or approve
10. After your medical professional reads the package of information the doctor will decide whether she wants to agree with the plan of care or whether we need to go in a different direction

After the rehab coordinator takes a look at it and make sure that everything is in the correct order and everything is supposed to be the way it is they will submit it to the rehab Department over at your insurance company your insurance company then has 30 to 60 days to look over the whole claim and decide whether they want to approve it if they have any questions within the 30 or 60 days they have the right to submit the request for information back to the durable medical provider and then the ATP will come back out to you or talk to you over the phone and try to get that information however if everything is looks how it's supposed to be the insurance will approve the request or claim then they will send the money to their optimization Department which will then send the money over to the durable medical provider like adapt health once it have health has the money then they will submit it to their purchasing department for order
13. Space about 6 to 8 weeks later after everything is approved by the company then your parts are delivered the rehab technician or ATP that has been working with you throughout this whole time will assemble the piece of equipment you have ordered make sure that everything is in working order and then delivered to your location once they deliver to your location they will make sure that it is appropriately fitted for you and that you make sure that you know how the piece of equipment works and that you know how to operate it this completes the process
