    "Methotrexate is a first line treatment for RA and other autoimmune diseases. It is an immunosuppressant meaning it suppresses the immune system. Methotrexate is a folic analog and works by inhibiting dihydrofolate reductase. Dihydrofolate reductase is an important enzyme in the synthesis of nucleotides."

    Thank you! I’ve been taking methotrexate for 23 years. During that time no doctor told me this or explained anything about side effects or why I should take folic acid. I was never told about problems that might arise if I got pregnant or any adverse reactions that might arise in my body. I’ve had several rheumatologists and now feel that my care was inferior or at least minimized. You’ve enlightened my understanding of methotrexate, which is far beyond of what I’ve read in the information sheets pharmacies have given me during these past 23 years. I really appreciate what you have shared with me in this video.

    should also put in a side note that it can have negative interactions with aspirin. I've been taking this med since I was about 18 and I only found out now at 32 years of age.

    Very good explanation. I’m trying to understand this because my Rheumatologist suggested methotrexate for my autoimmune disease(s), but i have MTHFR gene variants. I was told by another doctor that I should take l-methylfolate because I don’t metabolize any folate (is that even possible?) Could I have serious side effects from methotrexate even if I take folate with it?

    always thank you for you great work~~ I have a question. MTX can inhibit 'thymidylate synthase', too? In my text book , 5-FU inhibit that enzyme.

    Lots of what is explained here is a compilation of text book theories and arguments that have been put forth to explain the mode of action of methotrexate. The validity of these arguments is actually questionable as anti-inflammatory effects of MTX can be observed for concentrations that are below these needed to achieve the pharmacological effects described here. Also, this video ignores the fact that the mode of action of low-dose MTX (regimen usually used in the treatment of RA) relies upon metabolic transformation of MTX and that its anti-inflammatory effect is at least partly dependent upon release of extracellular adenosine and stimulation of A2AR on immune cells.

      Every rheumatologist I ever had recommended for ME not to take folic acid on the day(s) I take methotrexate , but I think that recommendation changes depending on the schedule, dosage and/or mode of administration.

    My Doc let me have 3 pills of 2,5 mg TWICE a week (3 Monday & 3 Tuesday) is this normal? I am diagnosed with Ra.

    Question. 2:18. "methotrexate reduces levels of methionine and this results in a decrease in methionine synthase activity"
    1. A decrease in 5 meythl THF would therefore logically cause an overload in the methionine cycle since more enzymes (methionine sytnthase) would be available.
    2. How does methetrexate cause a decrease in methionine levels? By point #1, it should increase the levels of methionine.

    Can methotrexate mess with other dividing cells we dont want it to mess with? Epithelial cells for example?

      Yes, it's non-specific in action. It's structure is very similar to dihydrofolate, so it will interact in the same areas as dihydrofolate, making it very non-specific and very toxic.

    So far it’s almost killed me and then now has been the only drug to reduce my swelling.. hard drug to handle and yet a wonderful drug in treating RA

      Yes I agree. My life has become full of fatigue and it’s been hard to manage that, but the drug has helped me.
