  • The Tyrant you are treating will kill 2 million people when cured. Do you treat him? Why? What ethical principles can justify your decision? House MD Season 6 Episode 4 The Tyrant was a whopping good episode to react to. Thanks to Amar Begic for the recommendation!
Without giving too much away, in it, I discuss:
    Dangers of Air Bubbles
    Agent Orange
    Dioxin Poisoning
    Systemic Lupus Erythematosus (SLE)
    Scleroderma
    Blastomycosis
    Four Ethical Principles
    Medical Ethics
The Tyrant was one of the House MD series' finer work, that's for sure.

13:00 - My read on the scene isn't that Dibala says that the specific air bubble (shown briefly) in the syringe will kill him; in fact, he grabs her arm so fast I doubt he saw the contents of the syringe. I think he's just saying it in general - like "toss some air in that injection if you want me dead". Additionally, Cameron doesn't actually attempt to clear the air bubble from the syringe before injecting the contents, and he has no negative reaction to it, so I'm not entirely sure that it's as big of a medical mistake as it appeared. Just my read on how they meant it though /shrug
17:10 - I swear this is the #1 thing TV gets wrong: shocking a non-shockable rhythm (especially asystole). Almost every doctor reaction on any series points it out at one point or another. Maybe it's just that the paddles are somehow seen as more "dramatic" than CPR and adrenaline? Considering how much time and effort shows like this spend on trying to get really obscure medical stuff correct, the least they could do is get this correct!

      Hi Echelar,
Thank you for your takes here and a very interesting perspective! I hadn’t looked at it that way because when the camera focuses on the syringe after looking at Cameron it feels very intentional that the air bubble was there. Also I think that when she injected it, she didn’t put it into his IV, she put it back into the vial it was drawn from.
Either way, Dibala is such a badass to say that while she has all the power stood next to him. What a character

It's quite the dilemma that Chase found himself in. Let one person live and risk the lives of hundreds of thousands or take the life of one person to save hundreds of thousands. Not an easy choice to make. More of a "damned if you do, damned if you don't" scenario. And the irony is that if one doesn't choose, that person will be criticized by both sides of the argument for failing to act.

As you know, this episode happens after the two part episode "Broken", so a couple of things happen between episodes.
House deals a lot with dilemmas, compared to Acceptance episode, this patient has no reason to have sympathy, and in the next episodes of this season, there's still things they deal about this case, so compared to most of the episodes, this is not really a one and done scenario.
The House's neighbor's phantom pain is very interesting, I watched a couple of medias that talk about this kind of problem with amputated limbs, but this show was the first time I saw about the mirror box, very interesting indeed

    • @DrSermedMezher

I thought the Broken episodes were very good. Glad I watched those before this one although it would’ve been nice to have the extra background from the middle episode!
Yeah when I saw the phantom limb pain disappear it was very moving! I’ve heard about it in theory but not visualised it like they do in this episode. I love how House raises awareness for treatments like these.
Looking forward to reacting to your next request next weekend!

5:34 Conflict for doctors.. they'll treat a dictator.. but real people are just given the boot. 😂 Okay full disclosure to strangers.. and probably over sharing.. but whatever. I'm an ex drug user. I say this because I never considered myself an addict. I suffer from major bouts of depression. I have since I was 7 years old. My parents, could never understand it, and I lacked a way to explain it to them. I ended up doing drugs as a teen because I was terrified of antidepressants. I had a family member who committed.. the long sleep.. because of them. Wether it was he quit taking them, or he needed different ones, I don't know. I didn't want to.. self delete. I've known that since I was 13, and walked up to that ledge, noose tied perfect, and tight and all. I'm very, mechanically inclined, so I made sure there was no possible way for it to fail. However, at that last moment.. there was a tiny spark in me, that would not let me do it. If I asked for help I was afraid that ADs would interfere with that spark. So I did the only thing my 13yr old mind who was desperate to not feel how I felt, could do. For most of my teenage years, I was on drugs... whatever I could get ahold of really. But from 16 to 18 was when the hardcore stuff came into play. Coke, the big M.. Not super often, I was still in Highschool and didn't have much money to spend. Double digits of times I did any of them. It was the ice mainly. Let me be clear. I never hurt anyone, I never robbed anyone. On paper, you can't even prove I've ever done drugs. I just got high, and laid in my bed being happy that I wasn't sad anymore. No criminal records, medical, nothing. I just got it any chance I got because, I couldn't stand how I felt.. all the time.. and at least for a few hours I was able to feel what true happiness felt like. At 18 I stopped.. no 12 steps, no talking to anyone, no rehab, no hospital, I just stopped. Nobody even knew, I hid it so well. However, one day, I accidentally severed a tendon in my hand, because I was high.. caused permanent damage to my hand to this day. After that, I came to my senses.. but the damage was done. To this day I still look like an addict.. even to myself. Despite 12 years of being clean of anything besides nicotine, caffeine, and alcohol now and then. But that's how I'm treated by society. What I was 12 years ago. 4 years ago, I started having chronic low back pain. (I worked construction) For the last 3 years, I've fought with doctors to at least give me the benefit of the doubt and help me, just so that I can live my life, work, and be a decent human being. I told them all I wanted was them to figure out what was wrong and either fix it or give me a doable way to live my life and deal with it. Even when I was on drugs as a teen, I never really used opioids. My aunt was a heroin addict. I saw what that did to her life and always stayed away from all needles, pills and the like. Also, they just weren't my thing. I told this to my first DR straight. To this day I'm treated like a drug addict and a criminal on death row for murder. I know it's a hard call for them to tell if someone like me is just trying to get drugs or if they just want attention or whatever. If any of the DRs I've seen had looked past my appearance and paid attention at all to what I've said.. they would know that opioids make me nauseous and vomit because of a stomach ulcer. I'm an introvert, so I don't want attention. I don't make a ton of money, so driving my car to appts every couple weeks is not only inconvenient, but wear and tear on my car that is potentially career ending for me if my car breaks bad enough. I'm 30, I like my job, I'm to the age that I would quit if I didn't. Probably couldn't get another if I did because of my appearance.. but also, If I wanted drugs, I could find them easier, cheaper, and better, than what I could get from a doctor. Also, I'd be able to work longer hours to afford them and make more money. The list goes on and on of why people are wrong about assuming I'm an addict based solely off my appearance. Wanna know the reason most drug addicts never recover? I know. Because society doesn't care about them at all. Some.. not all.. but some people.. they made a mistake, for reasons people don't even stop and consider to listen.. and would probably never understand.. hell sometimes the person themselves don't understand.. and society is "perfect".. and we're not. To them you are taking up space, breathing their air, and buying their food. Regardless of the reasons, and how hard you've fought to fix your life. You will always be trash and should just get back on drugs and.. OD. That is the world. It's crushing. Explain to me why I shouldn't just get the drugs I need for my pain to do my job and live my life off the street.. don't worry, I'll wait.

I love this episode, too. What I also appreciate about it is that Chase and Cameron did face consequences for their moral choices. At this point in the show, Chase and Cameron were in a happy relationship. However, Cameron couldn't accept what Chase had done. At one point, she says they need to move away, so Chase can be away from where this happened - that it was House and the environment that caused Chase to do it. But Chase was perfectly confident that he had made the right decision and wanted Cameron to support his decision. The situation was irreconcilable, and their relationship fell apart.

I’m a layman with two pop culture voices in my head. One is of the West Wing character Abby Bartlet who is consulted by her husband the president about what to do about a critical fifteen year old patient who is the son of the leader of a fictional terrorist nation who has been referred to the Americans for emergency services. She cites Samuel Mudd, who set the leg of Lincoln’s assassin John Wilkes Booth, who was tried and convicted for treason, saying that regardless, you set the leg. I haven’t had a chance to ask to ask a professional in the ten years since I’ve seen this, but In the extreme position that chase is in, what do you you think are his obligations as a doctor and his obligations as a member of a global moral community. Are there notable examples where circumstances compete with the oath bound ethical parameters of the job? Not necessarily asking for personal anecdotes but I noticed that a lot of comments had responses, so if you have heard of relevant cases on the topic I’d love to know about them.
