The Coruscant Guard pretty much stayed the same, however ARF troops and such were put into regular phase II armour (Why Hound has that helmet). All clones (that we know of) that were in the rebellion have the last known unit they served's icon, unless we know that they went back into normal life

In legends, yes. In canon, no as 1032 BBY is when the sith empire fell and therefore he must have died before that so c.1032 BBY is the year of his death as we cannot use the legends death date as that is not canon
We only know that they are extinct come 31 ABY so it's hard to say for sure what killed them, but given that they would be around 136 years old if they survived into 31 ABY, my best guess would be old age as you said

We only know that they are extinct come 31 ABY so it's hard to say for sure what killed them, but given that they would be around 136 years old if they survived into 31 ABY, my best guess would be old age as you said
