    Question 1 option 1 I find to be strange because it would be extremely rare for the pt in a hospital to be taking his or her own vitals before meds. So even though it is an illogical statement, should we just assume that it is true for any med questions since the overall statement expresses safe practice?

      Hi Gary, With teaching about meds, it's very common to teach them to take their pulse before they take a med that affects their pulse (digoxin). Remember that teaching and discharge planning starts as soon as the client enters the hospital. And we have to teach them on how to take the medication correctly. When to hold it, when to call the doctor. So, it's actually very applicable to real life. Then when they go home they will know what to do.

    hi, for a priority question, would you pick a potassium that is out of range or sepsis/infection?

      It totally depends on the symptom data presented. But in general if a client's potassium is critical AND they are showing signs of a life-threatening dysrhythmia then I would choose the extremely abnormal potassium level/deadly dysrhythmia over someone with a high WBC count and mild fever. However if the potassium level is only mildly out of range + no other symptoms, against someone who is starting to show signs of infection, I would choose the unexpected infection client. Hope that makes sense!

      There's no pdf copy. I think it's good to get the physical copy to write notes and and to refer back to pages. And there's over 400 images and it's all in color.
