Hmm...so when you cut the threads the first time, and when you made the adjustments to enlarge the thread, did the spindle and cutting tool pick up where it left off? I just assume that when you finished cutting the thread the first time, the spindle would not be able to realign itself with the thread cutting tool the second and third time you cut the part.
Is the hole & thread tapered or straight?

That’s a good question, yes it knows exactly where it is and picks the precise spot to recut.
