If u are in cruel diff then its a quest UI bug. The item will not drop.

@@danleecallender3 The item is not suppose to drop in CRUEL mode. Its a glitch for giving you quest UI.

I don't believe it's a bug, but rather a matter of less than 100% drop rate.
