You have to twist it if you want a genuine pressure bandage. It allows you to cut off the blood flow. It's basically like a tourniquet made out of gauze or bandage.
There are much better products,(e.g. Coban, Coflex, or an Ace Wrap), than Kerlix for use in providing the pressure portion of the dressing. Kerlix just doesn't have that much stretch. If you wanted to stick with a rolled gauze product use Kling instead.
