I watched this video numerous times but I could not understand some of it. At 51:30 , I could not understand the pronunciation of the word between ",,, expected interaction and you don't ,,, " . So I searched transcript and it says: ,,, it's just an expected interaction but you don't actually tell the person how you are. I hear "but" as "by". Could you please explain to me why ? And could you kindly explain the exact meaning of " you don't tell the person how you are ". Thank you.

Madam what is the difference between I have a fever and i have fever ? Why do we use “a” before fever when fever is uncountable because we can not count it one fever or two fever , right. Many other such examples are I have a headache I have a stomachache I have a cold Why do we use “a”????
