Don't forget crafters, real pearls have small drilled holes. But u can use a bead reamer to make holes bigger & always use a big eye needle. This is a lovely design though. I personally use faux pearls like in this video & that look fine

Swarovski has some really good ones) but I prefer flat-backs when doing something like this. They sit a bit better with less 'puckering' in the fabric underneath.
