Home First Approach in Cheshire East

In Cheshire East, we have a Home First approach to help people remain in the place they call home for as long as they can.
We also help people return to their home as soon as they are able after a stay in hospital.
Health, social care and voluntary services work closely together to make this possible. Delivering the right care at the right time and in the right place is fundamental to improving the care and wellbeing of our local communities.
