I have been struggling for years to create tables of contents automatically in Word and I would always give up feeling frustrated and end up doing it manually. Just so you guys know, pressing "update entire table" will get rid of any formatting, so don't format it (bold, italics, indentation) until you've got all your headings how you like them.
