    Keep in mind, students --- most people needing a C-spine x-ray are in pain and cannot extend the head back far enough to do the fuch's this way --- you can have them tilt back to look at ceiling (about 10-15 degrees then angle the tube cephalad approximately 23 degrees (an old tech taught me that angle and it freakin' works! Just keep the CR parallel to the MML) --- also - I'd never do a fuchs first --- do the open mouth first because not every patient needs a fuchs - just the ones where you can't see the entire dens/odontoid. Happy shooting!!!

      Fuchs is a wonderful method of understanding anatomy and how to visualize it. It's otherwise useless in the sense that nobody takes these images regularly.

      True the Fuchs is not needed on most cases but if so the magic angle is 37 degrees if the IOML is perpendicular to the IR.
