Actually, when you're not in the nightmarish Otherworld, you're in the Fog World. In the Fog World reality becomes distorted and the flow of time and the laws of physics can change. The Fog World usually appears sparse and abandoned with the exception of monsters. In Silent Hill 2 the monsters are representations of James Sunderland's repressed memories of the murder of his wife Mary, his sexual anxiety, and wish to be punished for his crimes; Angela Orosco's suffering due to years of sexual abuse from her own father; and Eddie Dombrowski's murderous thoughts towards everyone that mocked him in the past are all factors which helped to create new monsters

The game always felt like a before and after. Before, when it is just abandoned, and after when it has decayed
Kinda like how Centralia, Pennsylvania had a mine fire that wasn't fully extinguished
The "Breathing" represents Mary's illness (That she could barely do that). It always creeps me out...
Hearing this after the relatively calm journey through Rosewater Park and the bowling alley immediately refills me with dread.
