it is easy to performe in optical set up or privet clinic but not in the hospital or opd where pt flow is more than 20 per day
And what is the maximum prisms we should use for binocular balancing.
So basically in binocular blancing we are checking the accommodation is present or not?
Till image dissociates.. start with lesser value. NRA/fogging is used to relax accommodation but binocular balancing is used to check whether accommodation in both eyes are equally relaxed or not
The advice from your senior optometrist is not entirely true. Absence of binocular balancing might not be the main culprit for the complaint of Binocular discomfort during near task which she does not feel if same task is carried out monocularly. You need to also investigate the binocular vision system. Cover uncover, both unilateral and alternating, at near , PFR and NFR, flippers test and the vergence facility test.
Sir tq for the vedio. It should be done only for spherical power only. Or combination of spherical and cylindrical power.
It can be done to all prescriptions but effective in patient with accommodative spasm, pseudo myopia, situations where cycloplegia not possible..

It will done only for sphere. Even if it’s cylinder on monocular subjective refraction. You need to fogg the eye with plus lens then do binocular balancing then defogging for sphere/ fogged lens..
so we only make change to sphere not to cylinder.
If you put prism in both eyes like Re 4 and Le 5 and one image will up and one will be down, if they said RE is blur and Le clear then wht will do ?
Blur the image of LE by increase plus power till both eye becomes almost equal blur. That is the point when accommodation in both eyes are balanced.

hello Mr Noor, after doing duocrome balancing too is it necessary to do this, or if we feel that that the patient might have problem for near after the glasses just under correct it, without doing the balancing specially in 30+ patients ?
Yes balancing is necessary.. duochrome will you in checking under or over correction. Even fogging help in relaxing the accommodation of both eye but it may not tell whether accommodation in both eye are equal or not.
It can even be done on pre presbyopia as they may also exhibit some accommodation
