I encountered this issue when finished the Mariner's final quest "The Great Hunt". Captain Avery totally disappeared from the town, i found her in a private test room by using console command
Yes u can Just fixed Avery glitch today and was able to switch back to survival in game using console commands. Got Destroyer of Acadia and another achievement because of it

Leave far harbor, sleep, and then return. Repeat till she appears. Sadly that's the only way.
and i still can have achivements? or this is like new vegas when you use the comand then you cant get more achivements.
Good question.I didn't test this, but I believe you can still get achievements if using the console.
