How Pulling The Wrong Lever Caused Nepal's Worst Crash In 30 Years | Yeti Airlines 691

  • This is the story of Yeti flight 691. On the 15th of January 2023 the pilots of an ATR 72 tail number 9N-ANC of Yeti Airlines were having a busy morning, they were constantly shuttling the ATR 72 between the airports of Kathmandu and Pokhara. The thing about flight 691 was that it had two captains on board, the pilot in the left hand seat was being familiarised with the pokhara airport. the pilot flying and the pilot in the right hand seat was the pilot monitoring, who was the instructor. Now the thing is that the airport at Pokara was quite new, the old airport was very small and now a new international airport had been built in the vicinity and pilots were still learning all the new procedures for the new airport. So with that on that fateful day, they set off from kathmandu in the ATR 72 with 68 passengers and 4 crew members on board. The plane took off and everything was normal the weather was good, soo good that they were in visual conditions all the way to their destination and nothing appeared to be wrong. As flight 691 got closer to the airport of pokhara they got in touch with air traffic control and dlight 691 was assigned runway 30 for the landing. Now we dont know why but later on in the flight they asked for that to be changed to runway 12. They had already landed on runway 30 earlier on in the day and maybe they wanted to try landing on runway 30 so that the pilot flying could get some experience with both runways. We dont know why they made the change that they did, if you have any idea as to why they made the change that they did then please do let me know in the comments below.
    Then at 10:51 am local time the plane was at 6500 feet and they joined the pattern for runway 12 just north of the runway, ATC at this point was able to visually identify the plane and from their point everything looked good. As flight 691 flew the pattern, the pilots slowed the plane down and brought out the flaps to 15 degrees and 46 seconds later they dropped the gear. They were getting this plane ready to land. In the cabin people were excited about the landing with many of them looking out the window eager to capture a view of the city of Pokhara.
    At this point the plane was just 721 off of the ground and were on the downwind leg . The pilot flying called out flaps 30 and the pilot monitoring just replied with “flaps 30 continue descent”. This is when the problems started to show up for flight 691, The propeller rotation speed of both engines started to fall and then before you knew it they were less than 25%, we dont know how low they actually went because the flight data recorder does not record values below 25%. At the same time the torque being produced by the engines also fell to 0%. At 10:56 am and 36 seconds, a solitary little chime was heard in the cockpit, this was the master caution chime, but the pilots were just wrapping up the before-landing checklist at the time and didnt really notice the warning. As they made the left turn to get on to the base leg for the approach, that is they were now flying at a 90 degree angle to the runway and they still needed one final turn to line up with the runway , the engines had both gone into flight idle mode. Then in the cockpit the automated call out for 500 feet was made and it was followed up with the sound of a click meaning that someone in the cockpit silenced the master caution warning. The plane was then put into a left turn of 30 degrees, the pilot flying felt that something was wrong, so she asked the pilot monitoring, who was the more experienced pilot in this case if the left turn should be continued, he asked her to keep turning. Once the turn was done the pilot flying felt that things were still not right and asked the pilot monitoring if the approach should be continued. He didn't feel like it should be and asked asked the pilot flying to add more power. But this didnt fix the problem with flight 691. On the ground the controller cleared flight 691 for the landing,

      It’s probably a error on the smaller plane that taxied out to the runway and nobody’s noticed, possibly heard something wrong and thought to taxi.

    In this crash, one of the passengers recorded a video from inside the cabin as it happened. It appeared that he was just posting a live video of the landing until things went horribly wrong

    I wonder if the pilot monitoring was not usually in the right-hand seat, and just used 'memory' to pull the 'further set of levers' from when he was in the left-hand seat (Flap control) but in the right seat that becomes the pitch control (feather)...

    Why there's still no voice assist whenever such a change is made in the cockpit, like Flaps configured, or Altitude set? This would help the crew to validate the changes. This can be silenced during heavy workloads, optionally. This can be done in a soothing voice so that the crew doesn't feel annoyed.

    I've seen a few on this. Training /Senior Captain Kamala KC is 100% responsible to my mind. Despite the difference in how the levers are moved, he moves the wrong TWO levers instead of the correct ONE. Doesn't verify the position. The PF twice tells him about the lack of power. He does nothing and still doesn't verify his lever positions. He does move to "flaps 30" but again doesn't look at the console or prop positions (apparently)
    They had good weather, good ATC, a good airplane, plenty of time. The workload was no more than there would have been for any other landing.
    For me, he completely failed in his responsibilities as a Pilot.
    I usually avoid "zero sympathy" conclusions but this seems unavoidable.

      But I agree with your last statement. Assigning blame to an individual (especially one who is deceased) does nothing to improve aviation safety going forward. How was it possible that this was overlooked? What aspect of training could be improved to reduce the likelihood of it happening again?

      As a pilot or co pilot they should k ow every inch of the controls of that plane. In the same way a train driver should know the controls of their train.

      They were both captains? He says it at the beginning of the video. He was monitoring as she had very little experience on the new runway/airport. Was gross negligence by the male pilot in my opinion. A major one at that.

    Oh, I remember seeing this on the news, and reading all the speculation about what might have caused it. I don't think anyone had "accidentally reduced rpm instead of extending flaps" on their bingo card. Thanks for covering this one!

    I remember this incident very well mainly because of the two videos that went viral on twitter. One of which was footage from inside the cabin and the other from an eye-witness on the ground.

    This isn't the first time something like this has happened in a high wing turboprop.
    Never change anything in the cockpit without a visual confirmation that the right control is the one in your grasp!! This change was ignored by the crew when the master caution warning came on and it was silenced by another ignorant flight crewmember. The pilot flying stalled the wing in a 30 degree bank. Airspeed is clearly displayed because speed is life and lift. They lost both.

    There's a number of aircraft accidents that are associated with pilots not bothering to look at what lever or button they are pressing or not looking at their instruments to see if their action does what they expect it to do. Or even when things go wrong, look at their instruments for clues as to what went wrong.

    Dude I saw a video of a passenger live streaming the landing, you can see him smiling and then realizing what was about to happen. He screamed twice, then the crash happened, and its all super loud but dark because I guess the phone was flying everywhere, and then you ca only see debris and flames. Horrific

    What I question is how experiment pilots can pull a lever, flip a switch, or whatever, and completely fail to associate that action with the fact that something completely different happening as a result.
    You pull the (not) flaps lever, flaps don't deploy, and you lose power. How is the first thought anything but, "What did I just do?"

    Could a pilot please tell me why the feathering lever is in that location at all? Is it common to adjust the feathering in a normal flight? If so I'd be interested to know what it's used for. If not, then a lever with such dier ramifications should probably be placed somewhere more secluded, surely?

    To me it feels like letting the airspeed decay, despite a lack of power, shows that they weren't flying the airplane. They weren't following the basic principles of flight. They didn't appear to be aware of the aerodynamic characteristics of their aircraft. Which leads me to think that poor training is probably a big factor.

    I was born and raised in Pokhara and moved to Seattle in 1997. This flight is a sad incident that could have been easily avoided. Nepali pilots are extremely competent and able to fly in one of the world's most difficult terrains. It is hard to believe he made such a simple mistake. At the end of the day we are humans.

    Why don't they have a audio warning when the auto feather is engaged out the speaker? A single "Auto Feather" should sound. I always wondered being a science fiction geek why planes don't do more voice audio warnings? Will A.I. be introduced into aircraft as a advisory warning system only one day?

      One reason is not wanting to add more lights and bells to an already busy environment. Another, probably bigger reason is how much trouble (expense) it is to incorporate a new feature like that. Testing, evaluation, updating manuals and procedures, training....

      I kind of disagree on the music thing. Not sure if it's just my computer, but I find on a lot of YT videos that the audio cuts out completely during pauses in speech. There's a slight pause before the audio starts up again, so it's like the audio is muted and the speaker starts talking again slightly before unmuting. Quiet background music prevents it from happening.

    I am beginning to wonder, if it might have played a rule that de FO was also a captain. Normally as a captain you sit in the left seat of course, and when you do that for some time, you are used to that seat and the positions of buttons and levers from that position. You can find them (almost) blindly. However now that captain sat on the right seat, in which he might have been sitting for the last time many years ago. He was used to reach out quite far to move the flap lever, however from the right seat it is the very first lever to his left. It is just a thought. How do you think of this?

    What was the wind direction at the destination aerodrome? Planes will not land with a tailwind unless they have no other choice.

    With excellent visual conditions and the captain familiarising herself with the new airport, just wanting to get some experience with the different runways sounds like a good theory for why they changed. Seems like the perfect time.
    I am curious about potential issues a function to prevent this mishap could cause.
    Panic-slamming the throttle on both engines to full could have a feature to un-feather the propellers.
    but there are surely some problems it would cause, if just stickier levers or mechanical complexity.

      I'm not a specialist, but I can foresee at least one problem with such implementation. Say you have an engine failure, so you need to feather the damaged engine and go for an emergency landing. While in landing configuration (low altitude, low speed, flaps extended, landing gear down, so lots of drag), you have to do an escape maneuver (go around, wind shear, or TCAS), and reach for both throttles by mistake. If the damaged one becomes automatically unfeathered, the drag induced by it would add to all the drag already present and asymmetric thrust and could bring the plane down in seconds.

      ​@rodolfoptx If the engine has had its prop feathered due to engine failure, the engine has also been completely shut down. You can move the prop leaver all day long and nothing will happen.

      @@stevewhite3424 Of course, the engine won't do anything if it's shut down, but if the prop is unfeathered automatically by moving the thrust lever, as the main comment proposed, it would add back that drag you were trying to avoid when you feathered it in the first place.

      @@sriracha9729 Feathering a prop means changing the pitch angle of the blades to a neutral position, close to 90º: they won't make any thrust (speed you up) with the engine running, but also won't induce drag (slow you down) with the engine stopped. A great way to understand this is to put your hand out of a car window while traveling at highway speeds: when your palm is against the wind, you'll feel some pressure against it (unfeathered prop), but when you turn your hand sideways, that pressure is greatly reduced and you'll even feel some lift instead (feathered prop).

    I'm not sure if I agree that "taking the wrong lever" is unheard of... I was flying gliders for 15 years and I have seen people mistakingly pulling flaps instead of airbrakes. One of those occasions resulted in a crashed glider at the end of the runway... No injuries luckily...
    I have also seen people flying around with gear down just to don't gear up on the downwind...

    I can see this as both scary because it’s so simple of a mistake with such drastic consequences, and yet also as reassuring, because when you explain it it sounds like it’s almost unheard of because it’s like mistaking pulling the emergency brake with putting the car into reverse or something. It’s a minor mistake with huge implications, sure. But it’s just not a mistake drivers normally struggle with each time they drive a car. Thanks for the clear and un opinionated breakdown.

    still here and notifications are active. for this video, can you even imagine how cuold someone move the incorrect lever? happened down here on LAPA's flight with a crew not fit to fly (or even drive)

      Oh my goodness. The circumstances of LAPA Flight 3142 make me want to scream. Absolutely disgusting how the airline dispatched faulty aircraft and taught pilots to ignore warnings, on top of a flight crew that prioritized smoking and ranting over doing their dang jobs. How they were allowed to be within eleven miles of an airplane cockpit baffles me.

      ​​@@MisterKatz Yes it's true. Only 2 fatal accidents in 2023 in Commecial Aviation, not General Aviation. No fatal passenger jets crashes recorded in 2023 (not sure about Russia given the sanctions). The two crashes were both turboprop aircraft, Yeti Airlines in this video and Manaus Airlines in Brazil in September.

      There were 2 plane crashes in Brazil killing over 25 people in total, and a few other serious incidents like the firefighting 737 crashing in Australia, the ural airlines a320 ditching on a field, and a rogue pilot attempting to shut down the engines of a horizon air embraer mid flight

    The L3 FA2100 FDR does not record that many parameters, not sure where you got your info for that… and since you said “only” 187, you’d be surprised then how many are actually required per federal regulations… 😅

    It seems like there should be an added safety feature present in this airplane when the propellers are feathered, whether on purpose or otherwise, there should be a flashing light on the instrument panel, informing the crew that the propellers are feathered just like the stick will shake when there is an impending stall going to occur in this situation described here propellers were feathered and there wasn’t enough time or thinking available for the pilots to diagnose that. It was a problem with the propeller being feathered and there by not having any thrust although if they had been thinking, they should’ve figured that out mentally but let’s assume that they were Stressed over how to land at this alternate runway andwere distracted from making an appropriate conclusion about the lack of power when they put the engines back to full power. A bright, red, blinking warning light should exist when the propellers are feathered.

      I imagine if he was suicidal he could have just nosed it into the ground. Relying on your copilot not noticing the feathered props seems pretty unlikely.

    Normally a plane can glide down to the runway without engine considering the altitude and distance. Wondering why this plane just fell right down.

    And this is exactly why I can't understand why the fuck isn't there a cockpit AI monitoring what is being done, and while NEVER interfering with actual actions, nonetheless calling out "hey, I just noticed (you idiots) feathered the props (for no good reason) - you sure you wanted to DO THAT...?     BIT - Built-in In Test (or Built-in In self Test) is pronounced the same as a single bit (i.e. a one or a zero) of data and not like "A byte of data" or "I took a bite out of an apple".
