  • Oklahoma's Living Choice Program is a SoonerCare (the state’s Medicaid program) initiative designed to reform the current care system for people living in long-term care facilities. The program aims to shift the focus to home- and community-based services instead of institutional care.
    The program caters to three targeted populations: (1) individuals with physical disabilities (ages 19-64), (2) older adults with a long-term illness (age 65 and older), and (3) those with intellectual disabilities (ages 19 and older). Individuals or their families may apply for the services through the program if the individual has spent at least 60 days in an institutional setting, with at least one of those days covered by SoonerCare.
    Oklahoma’s Living Choice Program encourages independence and community life by providing supportive services allowing individuals with disabilities and long-term illnesses to live in an environment that reflects their preferences and enhances their well-being.
