Tips and tricks of VSD echocardiography (Ventricular Septal Defect)

  • Diagnosis of Ventricular Septal Defect (VSD) by echocardiography is easy for pediatric echocardiographic, but common associated findings are usually missed. So, never forget to search for the following items:-
    1- Additional VSDs
    2- Aortic cusp prolapse +/- aortic regurge
    3- Gerbode like defect
    4- Sub-aortic membrane
    5- Aortic coarctation
    6- Double chambered RV

    Recently we had a patient with a coa and the baby had a big vsd so that the coa turbulence didn't really show up. Later on it showed a really long transversal aortic Arche and an usual a. Subclavia. Also don't be sure if there is no turbulence in the descending aorta..but I am sure a lot of you know that already 😅

      Yes, you are right, COA, with large VSD, is very tricky as most of the flow is directed towards pulmonary circulation, making the peak gradient across descending aorta underestimated.
      So, any case with large or multiple VSDs, anticipate the presence of COA.
