Maybe you can't visit a sick villager with a friend from another island because the animal crossing villagers are just more responsible than us humans. I mean, this is just a theory a friend and I came up with, but acnh came out during the covid pandemic, and that virus only spread so fast because people weren't really taking it serious at first and kept traveling, even when they were sick, which lead to it being spread all around the world. So maybe the villagers are just thinking ahead a bit more than we did and refuse to let players from other islands enter their homes, so those players don't catch whatever the villager has and spread it all across their home island and whatever other islands they may visit. Again, this is just a theory, but given how the game came out during the pandemic, it would kinda make sense if it really was that way
I don’t believe so because illnesses were in previous AC games, and other people were allowed to visit your towns, but not allowed to visit sick villagers, and it’s not like it could spread because that’s just not how the game work.
Sometimes I think when I use the sneeze reaction around the villagers while it's raining, one of them will get a cold the following day. But it could be just a coincidence.🤧

Just watch out for lag when you have twenty or more of them on-screen...
