Investigation revealed deficient procedures and training by the crew of DDG-XXX, along with the fatigue of a crew which had been continuously at Quarters for the last 48 hours. This all combined with a civilian airliner crew whose dispatcher filed a route that took them inside a confusing and unclearly outlined DMZ. Air Hexagone had already been struggling after one of their airliners (Flight 314) went off the end of the runway in Kai Tak and caught fire the previous month. That incident remarkably led to little loss of life (only 1 fatality), but Air Hexagone had been near bankruptcy during the prior years (it was unsound to be flying empty 707s and L-1011s between Seoul and Tokyo to chase government subsidies) and the loss of their second MD-11 hull in two years finally pushed them into insolvency with subsequent furloughing of more than half its crews and staff. DDG-XXX was later involved in another incident off the coast of Java the next year when a torpedo malfunction during an exercise led to the ship being struck by a live Mark 48 claiming the lives of 8 sailors. Although she did not sink, the ship was subsequently laid up and remains in mothball at Puget Sound.
