Three tips for improving the ultrasound visibility of the needle tip (CVC).
If a needle has zero puncture resistance, there would be no tissue compression. Then, the techniques like Tip 1 are unnecessary. However, as mentioned in this video, it is necessary to advance the needle and probe alternately in order to confirm the needle tip position. So, the puncture method of "Continuously tracking the image of the needle tip from the beginning to the end" is like an armchair theory.
The best ultrasound setting should be determined in advance by performing a simulation with the ultrasound machine and needle that will be used in practice. Indeed, simulation makes perfect. I expect that the problem of black boxing of ultrasound machines will be solved in the future with...

How to quickly obtain a long-axis view of the needle during ultrasound-guided vascular access
Просмотров 9 тыс.4 года назад
In order to succeed in cannulation of blood vessels under ultrasound guidance, it is important to enter the blood vessel at the site where the vessel diameter is the largest, and further advance the needle tip within the blood vessel at the site where the vessel lumen is the largest. The short axis method has advantages over the long axis method for this purpose. However, for high-risk procedur...

Another Way of Locating the Needle Tip in Ultrasound-guided Deep Vein Cannulation
Просмотров 12 тыс.5 лет назад
Some people say that an acoustic shadow of a needle is a nuisance during ultrasound-guided vessel puncture. Indeed, this may be true for longitudinal imaging. However, an acoustic shadow can aid puncture under transverse imaging, as shown in this video. I named the technique "acoustic shadow tracing method" (AS tracing). Importantly, even when mainly using a technique other than AS tracing unde...
How to Safely Cannulate a Difficult Internal Jugular Vein on the First Attempt

Indeed, checking blood return is an essential procedure in CV line placement. But, putting too much priority on flashback can hamper efforts to visualize the needle tip on US screen. For a deeper insight into this topic, please go to doi.org/10.1186/s41100-016-0019-1 and view my article: “Ultrasound-guided cannulation of hemodialysis access”. Visualization of Guidewires by Ultrasound during Central Venous Catheter Placement
Просмотров 4,6 тыс.6 лет назад
To avoid complications by inadvertent arterial catheterization during central venous line placement, multiple approaches are needed. Imaging the guidewire with ultrasonography should be routinely added to those approaches. As demonstrated in this video, a large-diameter spring guidewire offers great ultrasound visibility under transverse imaging. There might be concerns about using a large guid...
How to make a cheap simulator for endotracheal intubation using a video laryngoscope (McGRATH MAC)
Просмотров 3,6 тыс.6 лет назад
By utilizing a toy mask [Ogawa Studio, Japan] and a simple larynx model (a spare part of a mannequin simulator [Laerdal Medical, Norway]), the cost was reduced to 1/5 - 1/10 of that of commercially available simulators for endotracheal intubation. Cautions are described as follows... 1) As the mask is made of rubber, its teeth are flexible. Therefore, take care not to distort the simulator’s fr...

The recipe is as follows, but unfortunately, it seems that cuboid konjak is only available in Japan. 1. To make mock vessels, build tunnels through a hard, cuboid konjac jelly by pressing a large straw into it. 2. Place the jelly in a tray whose depth is approximately the same as the thickness of the jelly. 3. Pour water into the tray until the top of the jelly is submerged. 4. Remove the air within the tunnels by squeezing the superior surface of the jelly with fingers.
